Miner was stacking stone on a pallet by hand when miner felt a pull in wrist. It got worse overnight so miner was sent to the clinic. Miner suffered a strain of left wrist and was placed on restricted duty.

Worker was operating a forklift. He went to step down from the forklift and felt a pull in his left calf.

Operator was cutting stone using a guillitone cutter. When he cut the stone, his L hand was under the back of the stone. The stone fell backwards on his hand, causing a fracture to the middle finger of his L hand. No Lost time.

Unknown. Employee did not report injury to supervisor.

He twisted while operating a forklift and felt back pain.

employee said he was pushing a stone and he hurt his back.

EE was stacking stones and got his finger caught between two stones.

Employee was taking a stone off of the conveyor belt and was going to stack it on a pallet. It slipped out of his hands and when he tried to catch it, he caught his right pinky finger between two stones.

Was stacking stone from conveyor onto pallet and then felt a pull in his lower back.

He was walking back for his lunch break and he twisted his ankle.

Was picking up a stone and when he placed it onto a pallet the stone edge caught his left thumb. He has a laceration to his left thumb.

WAS STACKING STONES ONTO A PALLET AND ONE FELL ON HIS FINGER. DR DIAGNOSED A CRUSH INJURY. FINGER LACERATION & DISTAL FRACTURE.

He was catching stone, and when he finished moving a big stone... he felt a pain in his lower mid-back.

WAS CARRYING A STONE TO A PALLET. AND WHEN HE PUT IT DOWN THE FELT A PULL IN HIS BACK.

EE DID NOT REPORT ACCIDENT RIGHT AWAY. HE TOLD OTHER ON FRIDAY 6-27-03 THAT A PIECE OF METAL FLEW FROM HIS CHISEL AND HIT HIS CHEST/STOMACH AREA.

EE WAS ON THE GROUND WORKING, WHEN A STONE WAS THROWN UP OR OFF OF THE LOADER TIRE. THE INJURY WAS TO HIS RIGHT LEG.

EE'S LEFT WRIST/HAND WAS CAUGHT BETWEEN TWO STONES WHEN HE WAS MOVING THEM.

HE TOLD HIS SUPERVISOR THAT HIS BACK JUST STARTED HURTING.

HIS LOWER RIGHT BACK STARTED TO HURT. HE WAS DOING NOTHING DIFFERENT THAN EVERY DAY THINGS. HE WAS IN & OUT OF HIS LOADER HE SAID. IT JUST JARRED & STARTED TO HURT.

HE WAS MOVING STONE AND HE FELT A PULL IN HIS LEFT SHOULDER AREA.

HE TOLD SUPERVISOR THAT HE HURT HIS BACK WHILE AT WORK ON JUNE 12TH, 2002. (HE DIDN'T LET ANYONE KNOW, HOWEVER, UNTIL THE 14TH OF JUNE.)

HE WORKED ALL DAY IN THE QUARRY & HIS BACK HURT.

HE WAS CUTTING STONE & A STONE FELL & CRUSHED HIS RIGHT HAND. WE AREN'T SURE WHEN HE'S COMING BACK TO WORK. HE HAD TO GO SEE AN ORTHOPEDIC SUGEON. I WILL FORWARD YOU A COPY WHEN HE CAN COME BA CK TO WORK.

WAS DRIVING THE FORKLIFT & A STONE FLEW UP & HIT HIM IN THE BACK OF THE HEAD.

LIFTED STONE & HURT HIS BACK. HE TOLD US ON 2-8-02.

CUT TIP OF RIGHT PINKY FINGER OFF.

SMASHED LEFT INDEX FINGER--BROKEN.
