Employee was loading a bulk truck. The employee started winding up the loading boot when dust particles entered his eye. Employee was wearing safety glasses.

Employee was emptying the lime out of the bucket of a small front-end loader. While the employee was driving the small front-end lader, lime dust blew back into hif ace. He suffered a lime burn to his right eye.

Employee was driving a forklift with a load of pallets on it. Employee misjudged being past the corner and when he turned to the left, employee hit cement pillar causing him to bruise his abdomen on the steering wheel.

Employee was using a front end loader to transfer stone from the stockpile to the reclaim system. The stockpile was frozen and snow covered in several areas. The employee undercut the stockpile attempting to reach thawed and thus free flowing stone. The stockpile overhead sloughed off and engulfed the loader into the cab.
