Injured on 3/30/12 loading material into loader bucket and foot was smashed under bucket. The emp. was evaluated @ ER and no med. treatment was administered and the injury was limited to bruising only. On 5/17/12 the emp. returned to the Dr. and a walking cast was applied. We feel this now meets the definition of med. treatment, therefore we are submitting this report as required.

Injury reported to Safety Mgr. on or around 9/5/08 that his shoulder was sore. He did not receive treatment at that time. No single incident was to blame. He didn't start receiving treatment until January 2009. He notified us on 1/27/09 that he was receiving treatment.
Operating forklift, wheel went in a rut and steering knob on steering wheel spun around real fast and hit left thumb, breaking left thumb.
