BOOM TRUCK OPERATOR APPARENTLY SWUNG LOAD NEAR POWERLINE. EMPLOYEE LENDING A HAND TO UNHOOK LOAD WAS ELECTROCUTED.

Miner slid over side of dredge screen, slid down slowly so SRL did not catch. Caused contusion and neck / sternum strain.

Struck by falling object

While descending caged ladder, SRL lanyard hook on fall protection harness became caught on ladder cage. As lanyard extended from SRL it knocked off miner's hardhat then lanyard hook dislodged falling and lacerating miner's forehead. Miner was at ground level when struck.
