EE HAD PARKED THE TELEDYNE TRUCK NEXT TO TWO TAIL GATES FROM HAUL TRUCKS THAT HAD BEEN STACKED ON THE GROUND. AS HE BEGAN TO LOWER THE OUTRIGGER ON THE TELLEDYNE TRUCK, ONE OF THE TAIL GATES SLIPPED AND LAYED ON TOP OF HIS FOOT.
EE WAS OPERATING A FORKLIFT, UNLOADING A MOTOR CARGO TRUCK, WHEN HE DISCOVERED THE FORKS WERE TOO NARROW FOR THE TWELVE FOOT LONG CRATED FREIGHT. EE STOOD BESIDE THE FORKLIFT, UNKNOWINGLY PLAC ED HIS RIGHT HAND ON THE FORK EXTENSION RAIL. EXTENDED THE FORKS, AND BROKE AND LACERATED HIS SMALL FINGER TIP.
EE WAS DRIVING A LHD WHILE UNDERGROUND AND RAN OVER A ROCK BOLT. THE ROCK BOLT FLIPPED UP AND ENTERED THE OPERATOR'S AREA. PIERCED EE S BOOT AND CAUSED A LACERATION TO HIS SHIN WHICH REQUIRED STITCHES.
EE WAS OPERATING HT3057 HAUL TRUCK DRIVING DOWN MAIN RAMP OF MURRAY MINE. EE CLAIMS HE WAS IN SECOND GEAR AND IS UNAWARE OF WHAT CAUSED HIM TO ENCOUNTER THE RIB AT Z-2 190/51. HIS NEXT RECOLLE CTION WAS CALLING THE MECHANICS. THE ACCIDENT INVESTIGATION HAS BEEN COMPLETED AND SOME INFORMATION INDICATES THAT EE MAY HAVE BEEN UNCONSCIOUS.
EMP WAS DRIVING LOWBOY (UV 3086) ON THE RIDGE ROAD AND HIT A POT HOLE, JARRING HIS BACK AND CAUSING LOWER BACK PAIN.

CEMENT TRANSFER BECAME PLUGGED. DECISION WAS MADE TO DRILL WITH A 36' STEEL & S58F. THE LEFG & DRILL BECAME TO HEAVY TO LIFT & THE WINCH TRUCK WAS ATTACHED TO GIVE UP PRESSURE. THE PRESSURE CA USED THE LEG TO BECOME BOUND, WHEN IT CAME LOOSEIT STRUCK EE ON THE LEFT SHOULDER & HEAD.
EE WAS SERVICING THE 913 LDH, STANDING AT REAR WNEAR FUEL TANK WHEN A CONTACT SUPPLIER TRUCK LOST CONTROL OF VEHICLE BECAUSE OF SLICK ROAD CONDITIONS AND SLID INTO LOADER STRIKING AND PINNING EE BETWEEN UNITS.
EMPLOYEE WAS A PASSENGER ON KUBOTHA TRACTOR TRAVELING TO PLACE OF WORK. WHEN THE FRONT TIRE STRUCK A ROCK CAUSING THE BUGGIE TO DART AND STRIKE THE RIB. THE PASSENGER WAS THROWN FROM BUGGIE INJURING HIS LEFT ANKLE AND SHOULDER.

EMPLOYEE WAS DRIVING THE DRILLER'S 2 1/2 TON WATER FROM HIS BASIN PIT OT THE MINE ADMINSTRACTION BUILDING AT THE END OF HER SHIFT. SHE WAS TRAVELING TOO FAST AS SHE WENT AROUNDA CURVE ABOUT 1/ 2 MILE FROM TEH PATTANI GAP AND ROLLED THE TRUCK ONTO IT'S RIGHT SIDE.
EMPLOYEE WAS TRAVELING DOWN HAUL ROAD TO ORE STOCKPILES AND TRUCK BEGAN TO FREE WHEEL ON 4.5 GRADE. ALTHOUGH THERE WERE TWO RUNOFF RAMPS, THE EMPLOYEE CHOOSE TO RIDE OUT THE RUNAWAY. HE LOST C ONTROL IN A CURVE AND OVERTURNED THE TRUCK. EMPLOYEE WAS WEARING SEATBELT AND ROPS PROTECTED CAB.
DOING PAPERWORK IN CAB OF TRUCK HAUL TRUCK BUMPED IN FRONT OF TRUCK
INJURED WAS UPON A LOWBED STARING TO UNLOAD A CAT. WHEN THE LOWBED STARTED TO MOVE BACKWARDS DOWN A STEEP INCLINE. HE ATTEMPTED TO JUMP OFF AND IN DOING SO, SUFFERED A BROKEN LEG AND HURT HIS OTHER FOOT AS WELL. HE SLIPPED - LANDING BACKWARDS.
I,NAME TRUCK HA8 BEEN PARTIALLY LOADED WITH A LARGE ROCK ROLLED DOWN ONTO THE TAIL END OF HIS TRUCK. WHILE ATTEMPTING TO PULL AWAY, THE FRONT END OF THE TRUCK CAME UP OFF OF THE GROUND. WHEN T HE TRUCK BOUNCE8 BACK MAKING CONTACT WITH THE GROUND, I,NAME SUSTAINED INJURY TO HIS BACK.
I.NAME WAS ASSISTING IN REINSTALLING THE VIBRATING FEEDER IN POSITION WHEN HE PLACED HIS FINGERS BETWEEN THE FEEDER AND CHUTE CAUSING THEM TO BE CUT. I.NAME HAD SEVERAL STITCHES BUT WAS RELEA SED FOR FULL DUTY ON HIS NEXT SCHEDULED SHIFT 5/22/84
