Ee was replacing hoist cylinders on a CAT 777. The cylinder was hooked to a crane. Slack in the crane attachment allowed the cylinder to move, crushing the ee's right hand between the hoist cylinder and the fuel tank. The ee was diagnosed with a fracture to the right ring finger.

Process operator cleaned off level transmitter for RIL feed hopper. EE noticed the level in the vessel rising and exited the platform using the access ladder. EE made it to the ground and the vessel overflowed, causing hot slurry to hit them on the right side of the body, partially filling their right and left boots with hot slurry. The hot slurry burned their bilateral lower legs.
While driving the Getman flatbed on the lower Rodeo ramp between the 3620 and the 1600 levels the employee drove through a water puddle near the sumps and hit a rock that was covered by the water. The Getman bounced and the employee hit knee against the lower part of the operators compartment dash board.
EE was driving a remix truck when ee hit a pothole, causing ee to bounce in the cab and hit ee head on the truck cab. EE complained of pain in the neck, head, and shoulders. Seen at the ER, MRI negative for any acute injury, ee diagnosed with a neck sprain and put off work.
Poor road conditions on mine site
Ee was sitting in haul truck being loaded. Large boulder fell onto tail-board, jolting the truck forward and the ee hyperextended his left elbow. Seen at ER and put off work, diagnosed with a soft tissue injury.
Ee was sitting in a gator and a powder truck backed into it. Ee felt pain in neck and upper back, notified supervisor but did not wish to see doctor. Pain didn't go away, saw doctor on 12/12/14. Diagnosed with a whiplash type injury and referred for chiropractic therapy. Released to full duty.
Ee walking to buggy to leave work area; powder truck backed into ee, striking his back. Ee sought treatment 11/15/14. Diagnosed w/back strain. Seen again at ER on 11/18, diagnosed w/non-occupational gallstones. Surgery did not relieve back pain, seen on 11/25, put on restricted duty for back pain due to 11/12/14 incident.
Ee was watering the 4560 upper west griffin ramp. He turned at the top of the ramp and went back down. The ee was unable to maintain control of the equipment and the truck contacted the rib several times, coming to an abrupt stop causing injuries to the operator. Ee was seen at the ER and diagnosed with fractured ribs and a laceration to the back of his head. Ee put off work.
Ee was driving haul truck underground, hit bump in the road, causing her hard hat to come off, hit the roof of the cab, and then hit her head. Laceration requiring stitches and put off work after diagnosis of concussion.
While cleaning a hole, the lip of the bucket caught on the rib on the way down, striking a rock, jarring the employee in the cab. He felt pain in his upper and lower back. He was examined by a physician and placed off work pending follow up.

Ee turned off ramp and the truck did not come out of the turn, striking the rib, stopping the truck. The ee hyperextended his thumb. He was evaluated by a doctor and diagnosed with a strain to the thumb, given a brace and released to restricted duty.
Driver was on 3rd trip to pit, on 1st trip a break test was done, he was still in high gear and was unable to slow or stop the truck and ran into a pile of agg ore next to the underground mine wall.
A lube technician hydraulically raised the cab of a Caterpillar D300 lube truck. While placing the cab support safety strut in position, the cab suddenly fell striking the employee's hand.

Employee was servicing water truck at the end of shift. While holding up the engine cover and reaching for a container of oil, the heavy cover started to come down, pushing the employee off balance backwards. The employee jumped down awkwardly away from the engine cover, landing on his feet, from a height of approximately five feet. Fractured left ankle and right heel.
Emp was dumping load and the bed of the truck fell, jarring his neck. He was seen at the ER, diagnosed with a cervical strain and released to light duty for two shifts.

A piece of flex pipe fell from a forklift and struck the employee on the left shoulder. The employee fell to the ground. He was diagnosed with a fractured clavicle and an abrasion to the right elbow. He was released to light duty.

Employee was riding in a tractor underground on the 3720. He felt a drop of acid water hit his safety glasses and go into his left eye. He was seen at Elko Eye Center, given eye drops as a preventative measure, and released to full duty.
A hose/fitting became disconnected from a tank (acid delivery trailer)
A mechanic received an injury to his finger while unloading a raditor. Raditor was being positoned near the front of the bulldozer by a service truck crane when the mechanic inadvertenly got his finger pinched between the raditior and the bulldozer push arm. Injured mechanic was taken to the hospital for medical treatment.
EE was cleaning rock spill with skidsteer. Hit dump pocket with skidsteer stopping machine and bruising shin.
Employee was operating a water truck and hit his head on the radio box. Laceration required 4 stitches.

Fell approx. 15-20' to the ground from cab of overturned haul truck. EE comments to EMT's indicated he experienced a physical episode affecting his consciousness or orientation prior to event. Incident under investigation.6 broken ribs, punctured lungs, 2 compression fractures of L1/L4, strain to right arm, lacerations to face/head, and damaged tailbone.
Employee was riding on hot change bus. Bus hit a hole and threw the employee around in the seat. He was transported to NNRH ER and admitted to the hospital after a series of episodes involving his heart and blood pressure. He was diagnosed with back strain, hip strain, and cardiac bruising.
Emp was driving lube truck underground. A wire was sticking out from the rib, and the emp hit his forehead above the right eye on the wire. It caused a puncture wound which was cleaned at the emergency room. A drain was placed in the wound and the emp will follow-up before his next scheduled shift.
Rock from shovel bucket struck truck bed and jarred emp. Emp experienced immediate neck pain and was transported by ambulance to emergency room. X-rays of neck negative for fracture, emp diagnosed with acute cervical strain. Upon follow-up with doctor before next scheduled shift, emp placed on restricted duty.
The employee had turned into the wrong dump area, found a place to turn around and while in the middle of the turn, the steering wheel jerked and his shoulder was pulled and he heard it pop. He sought medical evaluation on 10-20-04 and was refered to a specialist. He was diagnosed with a torn Labrum on 11-08-04, and scheduled for surgery making the incident reportable.
The employee was stepping down off the deck of a scissor lift. The scissor lift lowered a little more and caught the employees left foot between the deck and frame. He was transported to the NNRH ER for evaluation and treatment. He was diagnosed with a fracture and placed on restricted duty.
Injured employee was standing in man-basket while hanging instruments against the back. While holding instrument in place with left hand, injured employee moved the man-basket with his right hand, crushing and lacerating left ring finger.
Traveling down ramp, truck driven by ee had flat tire. Loader reached under front of truck with bucket & began towing truck out of way. Truck slipped off edge of bucket, causing ee to be jerked around, hit head on controls. EE stated on day of accident that it just hurt where he had old injury. June 7, he complained of pain and dizziness - head & neck injuries.
The employee was driving a haul truck down the ramp, when the truck started gaining speed. He attempted to put on the brakes, but the pedal went to the floor. He steered the truck toward the rib, hitting the rib at least twice before coming to a stop. He was transported to the ER where he was treated for a 3" deep puncture wound to the lower left abdomen and taken off work.

HE WAS USING THE CRANE CONTROLLER WITH HIS RIGHT HAND & WAS HOLDING IMPELL ER W/LEFT HAND. WHEN IMPELLER SPUN FREE, IT SWUNG OUTWARD & EE'S LEFT THUMB WAS CAUGHT BETWEEN IMPELLER & FEED SPOOL PIECE, LACERATING IT. HE WAS TAKEN TO ER FOR TREATMENT & STITCHES WERE REQ
EE WAS CLEANING UP AROUND THE PLANT. WITH THE SKIDSTEER HE HIT A GUARD CAUSING ONE END OF THE FALL TO THE GROUND. AFTER PUTTING IT BACK INTO PLATE HE NOTICED THE SIGN OF THE GARD WAS OUT OF AL LIGNMENT. HE PULLED UP TIGHT TO CONVEYOR TOOK THE SEAT BELT OFF REACHED OUT WITH HIS LEFT HAND TO BRACE HIMSELF AND LOANED TO FORWARD AND HIT THE FOWARD LEVEL CAUSING HIS LEFT HAND TO BE PINC
