THE MINE RESCUE TEAM WAS PARTICIPATING IN AN UNDERGROUND MINE RESCUE EXERCISE IN AN ABANDONED MINE. AS THE TEAM WAS WALKING UP A STEEP DECLINE ON THEIR RETURN TO THE SURFACE, TWO TEAM MEMBERS COLLAPSED. ONE VICTIM WAS PRONOUNCED DEAD AT THE SCENE. THIS TEAM MEMBER WAS TRANSPORTED TO THE HOSPITAL WHERE HE DIED ON 10/23/02.

THE EMPLOYEE WAS WORKING FROM THE GALLOWAY WORK PLATFORM IN THE BOTTOM OF THE RODEO VENTILATION SHAFT. THEY HAD JUST FINISHED LOADING AND WIRING IN THE BLAST FOR THE END OF SHIFT. THE ORE BU CKET HAD BEEN CALLED FOR SO THAT THE CREW COULD RIDE BACK OUT TO THE COLLAR FOR THE BLAST TIME AND SHIFT END. Driver was on 3rd trip to pit, on 1st trip a break test was done, he was still in high gear and was unable to slow or stop the truck and ran into a pile of agg ore next to the underground mine wall.

Employee was energizing an airline, he turned on the wrong valve and that airline whipped off the hooks and hit him in the left lower leg causing a tib-fib fracture.

Miner was in process of cleaning shotecrete sprayer; when he released the quick-vic, material under pressure struck miner in face. Miner sustained corneal burn to right eye.

Fueling small pump engine. Gasoline spilled on hot exhaust, flaring up and burning employee. Blistered hands, reddened neck and face. Eyes okay, wearing safety glasses.

Was moving oxygen cylinders to a cylinder rack in the underground shop when a cylinder fell over and he tried to stop the fall and pinched his finger between the cylinder and the rack. Cut finger received 7 sutures.

EE WAS REMOVING SOME WIRE THAT WAS STUCK ON THE FORKS OF THE FORKLIFT. WHEN THE FORK CAME FREE, IT PINCHED EE LEFT THUMB BETWEEN THE FORK AND THE BACK BOARD OF THE FORK LIFT.

EE WAS DRILLING FOR ROCKBOLTS AND HANGER BRACKETS. THEY WERE IN FULL BODT HARNESS, TIED OFF INSIDE A MAN BASKET, LIFTED BY A FORKLIFT. WHILE COLLARING HOLE, THE JACKLEG BUCKLED AND KICKED BACK PUSHING HIM TO THE SIDE HITTING HIM IN THE FACE. HE HAD NO BROKEN BONES BUT DID HAVE 2 STITCHESAND A SWOLLEN FACE. HE WAS KEPT IN HOSPITAL FOR OBSERVATION.

THE EE WAS ROCK BOLTING STANDING ON MUCK PILE OFUNEVEN MATERIAL. HE STEPPED BACK TO PULL THE BOLTER OUT OF THE HOLE ONTO A ROCK & TWISTED HIS RIGHT KNEE. HE REPORTED THE INJURY TO HIS SUPERVIS OR BUT DID NOT GO TO DR UNTIL 1/17/01. HE RECD A FULL DUTY RELEASE TO RETURN TO WORK WE WERE UNAWARE HE WAS CONTINUING TO HAVE PROBLEMS WITH HIS KNEE UNTIL WE WERE NOTIFIED ON 5/4/01 BY WORKE

EE WAS PULLING ELECTRICAL CABLE AROUND A STEEL COLUMN. HIS THUMB WAS CUT BY A SHARP EDGE ON THE COLUMN.
