EE was working with another EE to jack up the cab of a 980 loader in order to access a hydraulic pump. The loader was being raised implementally with two jacks. During the lifting process the EE noticed a wire was caught and reached in to free it. The jack slipped and caused the cab to drop and pinch the EE's arm.
Employee states he was helping another employee take the bucket off of the skid steer. The pin would not release; the bucket was then raised up so the pin could be hit from the bottom. Employee hit a lever which caused the bucket to fall, striking the employee's shin, ankle and foot.
Employee was operating the haul truck unit. After having it loaded with dirt he was driving around a turn at the north end of the pit on the haul road. As he rounded the turn he stated he felt the shift in the rear end toward the inside of the turn and as he tried to compensate the truck rolled over.
Employee was moving rocks with loader, large rock fell out of bucket and loader ran over it causing the loader to shift and he impacted his head and shoulder against side.
AFTER OPERATING BOBCAT ON 5/17/05 EMPLOYEE WAS SHOVELING & SLOWLY HAD ONSET OF BACK PAIN. EMPLOYEE DID NOT NOTIFY SUPERVISOR UNTIL 5/31/05 AT WHICH TIME HE SOUGHT MEDICAL TREATMENT
EMPLOYEE FELT LOWER BACK AND HIP PAIN WHILE OPERATING THE LOADER IN THE WASH PLANT YARD (BOUNCING IN SEAT). HE VISITED A DOCTOR WHO FELT THAT THE CONDITION WAS DUE TO BODY ALIGNMENT VERSES A D ISK PROBLEM. ON 2/2/04 THE EMPLOYEE WAS TREATED FOR A BULGING DISK BY ANOTHER DOCTOR. THE EMPLOYEE REQUIRED SURGERY AND THE INJURY IS NOW BEING CONSIDERED WORK-RELATED.
EE WAS DIRECTING LOADER TOWARDS TRAILER, WHEN DRIVER OF LOADER PUT ON BRAKES, LOADER SLID ON ICE CATCHING EE'S HAND.
