Fall to the walkway or working surface

Employee & supervisor were attempting to remove nipple of water valve on water truck. Employee using a pipe wrench on nipple while supervisor was heating 3"" 90% cast iron pipe elbow. The wrench slipped and employee fell backwards while twisting to break fall. Twisting of left lower leg resulting in fracture. The ground was level with no slip/trip/fall hazards present.

AFTER OPERATING BOBCAT ON 5/17/05 EMPLOYEE WAS SHOVELING & SLOWLY HAD ONSET OF BACK PAIN. EMPLOYEE DID NOT NOTIFY SUPERVISOR UNTIL 5/31/05 AT WHICH TIME HE SOUGHT MEDICAL TREATMENT
