Showing all 8Starter on maintenance truck would not disengage. Wiring melted causing smoke. The miner inhaled smoke, started coughing, became nauseated, and fainted momentarily. Evaluated by EMT and given Phenothiazine for nausea. Taken to ER for doctor evaluation, checked out fine. Sent home for two days to rest told to follow up with primary care doctor.
The LHD operator was driving when he ran over a rock. This action caused the LHD to bounce. The operator bounced up and hit his head on the canopy. Straining the muscles of his neck.
After just entering the mine road he pulled over to the side of road. He waited in front of his truck to the left side. A customer's truck approached the intersect and instead of slowing for the corner the customer cut the corner clipping the back of the employee's truck with the rear duals of his trailer. This pushed the truck into him knocking him to the ground.

EMPLOYEE WAS DRIVING ALONG BAYS IN HIGHTOP AREASAROUND 14-5. HE WENT OVER 3 CUTS AT AN IDLE WHEN HE CAME OFF 3RD CUT HE GOT A SHOOTING PAIN IN HIS BACK.
HE WAS CLEANING A ROOM WITH THE BLADE OF THE BUCKET FLAT ON THE FLOOR. THE BUCKET HIT A CUT IN THE FLOOR WHICH EJECTED HIM FROM THE MACHINE. THIS CAUSED A DISLOCATED FINGER AND SOME SCRAPES.
