Struck by falling object

EE was building a stand for the new Cage Paktor. Weld broke on stand and struck EE in head causing laceration.

Struck against stationary object

EE moved rail loading chute to close lid on rail car. When moving the chute EE cut right wrist. EE was taken to ER for stitches.

Over-exertion in lifting objects

EE was shoveling up a pile of salt by the crusher toward end of shift. EE's back started hurting and became worse over the next few days. EE went to DR 08/08/25 was given pain meds and placed on light duty. EE returns to DR in 1 week for revaluation.

EE slipped on a salt rock while pulling power cables to face panel. EE hyper extended left knee. EE was taken to ER for x-rays and full evaluation. EE was diagnosed with knee sprain. Given topical cream for pain and swelling. EE returned to full duty Monday 3/24/25.

Struck against stationary object

EE stepped on a chunk of salt and slipped, strained left knee. Went to DR 3/21/25 and 3/28/25 no work restrictions. Returned to DR 4/7/25 for MRI. Torn meniscus identified 4/9/25 after MRI was reviewed by DR. EE will be sent to Ortho Knee Specialist.

EE strained right shoulder while flat racking salt. EE was already under care of DR for left shoulder. No treatment was given or recommended by DR during November or December. EE returned to Dr for left shoulder appointment told Dr right shoulder was getting worse. DR gave steroid injection and prescribed Meloxicam.

EE accidently hit start button on car puller. EE's hand was caught between car puller and rope, pinching crushing injury. EE was taken to ER for treatment. EE had surgery 4/19/2024 to repair broken fingers, damaged ligaments, and amputation of tip of finger.

EE slipped in parking lot after work during snow storm. Parking lot had been treated/ cleared but remained snow packed/ ice due to below 0 temps. EE told supervisor EE was okay. EE contacted supervisor next day was sore. EE sent to DR, one week no shoveling. EE returned to DR one more week no shoveling.

EE stated EE's foot popped when walking down steps. EE did not report injury until 9/15/2022.

EE twisted left knee 8/13/22, did not report injury until 9/21/22. EE went to DR 9/22/22, no lost time still performing regular duty. EE reported continued pain went to DR 10/7/22 placed on restricted duty for 2 weeks. DR believes PT will resolve but recommended Ortho consult.

Fall down stairs

EE walking down headframe stairs, right knee gave out. EE bruised knee, minor scrapes. EE did not report until 7/22/2022. EE worked 2 more shifts before requesting DR visit. DR prescribed meds for swelling, no work restrictions. EE scheduled for follow up appointment.

EE tripped on forklift fork, fell on right shoulder. Pain went away, pain came back. EE went to DR 5-9-22 no medical treatment no work restrictions. EE referred to Orthopedic DR for evaluation. EE evaluated by Orthopedic DR, surgery recommended on 6-20-22. EE has no work restrictions or lost work days as of 6-20-22. Date of surgery has not been scheduled as of 6-20-22.

EE scrapped left leg on railcar 12/1/2021 worked remainder of week with no issues. EE called 1 am Sunday 12/6/2021 said leg was swollen. EE taken to ER no broken bones or infection just swelling.

EE slipped when going down steps, bruised left hip.

Bulk loader closing railcar gate with bar. Gate gear skipped track, right hand was smashed between the bar and the bottom of railcar. Bulk loader was taken to ER, diagnosed with hand contusion. Given a hand brace to wear, released to return to work 6/17/2021.

Operator changing broken bolt on undercutter auger. Operator pinched right ring finger when auger moved.

Miner felt pop in right shoulder when opening forklift engine compartment door. Also had a small sliver of metal get in right hand when cleaning tramp iron from magnet. Shoulder and hand were bothering miner, went to Dr 8/19/2020.

Maintenance Leadman using a grinder with cutting wheel to cut piece of stainless steel for a chute. Grinder kicked back and cut right hand behind thumb. Taken to ER for glue and stitches. Guard was in place on grinder, loss of control of grinder caused injury.

Bulk B loader missed rail car rung with foot when dismounting rail car ladder. Fell to ground and broke left heel.

Impact with Mini Excavator bucket while operating, X-ray negative, bruised shoulder.

Ice storm hit mine and surrounding areas in state. The miners were first to arrive on day shift all areas had not yet been treated with salt. Miner slipped on ice broke rib 6&7 left side.

LHD Operator ran over chunk of salt, LHD travel was disrupted. Seat came off mounting brackets operators hit right knee on operator compartment under dash.

Over-exertion in lifting objects

Pull in right lower abdomen while lifting a 50LBS bag of ANFO. Kept working, went to doctor on 4/25/19, Dr referral to surgeon. Surgery 5/13/19 return to work 5/14/19 restricted duty.

Inhalation of radiations, caustics, toxic and noxious substances

Starter on maintenance truck would not disengage. Wiring melted causing smoke. The miner inhaled smoke, started coughing, became nauseated, and fainted momentarily. Evaluated by EMT and given Phenothiazine for nausea. Taken to ER for doctor evaluation, checked out fine. Sent home for two days to rest told to follow up with primary care doctor.

Injury 5/27 changing roller. No lost time/medical treatment/restricted duty. 6/15 aggravated injury using hammer. Reported 6/18 sent to DR no lost time/medical treatment. Restricted job duties. 6/27 DR apt restricted job duties. Medical treatment 7/9 physical therapy.

Struck by falling object

EE was retrieving square steel tube from surface maintenance shop storage rack. The tube fell, struck EE's left foot causing left first metatarsal fracture, transverse, acute, non-displaced injury. EE returned to work, light duty in office. Follow up appointments are scheduled Jan 9th & 11th.

Changing hoist rope and lost control. Fiber rope attached to the end of the hoist rope. EE left hand was injured when the rope gave way and ee fell to the ground, injuring left shoulder and knee.

EE was walking to screen room floor from truck side. Another employee was opening the door from 3rd floor and ee tripped and broke fall with left hand, cutting middle finger on a broken hinge. EE was given stitches.

EE dropped a screw because it was cold out and fingers were cold. In looking for screw on ground, a gust of wind occurred and EE felt something hit EE in the right eye. It was determined EE had a corneal abrasion and conjunctivitis.

EE was installing a guide in the north shaft. EE helped lift a guide out of position and install a new one. Next morning had pain in left upper chest. Went to Dr., Dr. said ee had a muscle strain. Prescribed steroid.

EE was cutting timber shaft guides. The saw used would not cut the depth of the guide. EE used a sledge hammer to break off the end. EE reported that they felt a pull in right lower back, while doing this.

Employee was driving a forklift into the shed to dump a hopper of salt. When employee raised the hopper to dump it, a hydraulic line blew spraying employee in the right eye.

Employee was in the bathroom and did not lock the door. An employee opened door quickly and door struck left shoulder and pushed employee into the wall.

He was scaling shale with a loader. He drug the teeth of the bucket on the back of the shale. A small piece of shale hit the frame of loader sending dust and debris into the cab and into his eye.

Working on fire hydrant platform when the connections for the platform gave out. The platform side that gave away was foamed and wasn't able to see any bad connections.

Scaling the roof by hand with a scaling bar. As he was scaling a loose rock fell and struck the bar causing intense pain in right arm.

Employee was bringing down rail cars. Said he applied the brakes and they did not work. Jumped off railcar and struck the ground. Had sudden pain in legs.

Truck driver on our property to get a load of salt. He was loaded and pulled off scale. he got out of his truck. He collapsed and fell and hit his head on the gravel. Ambulance came and took him to hospital. They air-lifted him to Wesley Medical in Wichita. Last report was head trauma.

He was removing a guide by cutting it into manageable pieces and his hand got caught between two pieces of timber which cut his finger.

He was on his knees working on equipment. Was not wearing available knee pads. Said his knee started hurting and swelling.

Employee was leaving for Dr. appt. when he was struck by rock salt that was loosened by a piece of scroll that came off the hoist and traveled down the shaft.

While putting rope on car puller to advance rail car, EE was wrapping rope around puller. He noticed the rope was climbing so he reached down to guide rope. He also noticed slack in the loop and rail car was not moving. While adjusting rope, finger got pinched between layers of rope and he smashed his finger. He broke the tip of his pinky finger.

EE unplugged cable which was corroded. He strained his lower back by wiggling and pulling the plug harder than normal.

Ladder slipped as he was climbing down from 1600G. He jumped to the ground from second or third step. When he landed he sprained his right knee.

Walking across the parking lot. The parking lot was snow packed and icy. He slipped and fell twice, injuring his right shoulder.

Employee was standing on pipe lifting pipe to others. The employee twisted knee. Employee waited 1 week to report accident to company. Didn't want medical help at this time. Waited 2 weeks and was still having pain so was sent to doctor and was diagnosed with ""water on the knee"" and received a shot.

The EE lifted the top portion of the freight elevator door with his left hand and used his foot to push down on the bottom portion. The door became un-wedged quickly and resulted in his left index finger getting caught in the door mechanism. He was given Loratab and released to work.

Miner was replacing a bugduster auger that had fallen out of machine and lacerated his left middle and ring fingers at the second knuckle.

Struck by powered moving object

After just entering the mine road he pulled over to the side of road. He waited in front of his truck to the left side. A customer's truck approached the intersect and instead of slowing for the corner the customer cut the corner clipping the back of the employee's truck with the rear duals of his trailer. This pushed the truck into him knocking him to the ground.

HE WAS CLEANING A ROOM WITH THE BLADE OF THE BUCKET FLAT ON THE FLOOR. THE BUCKET HIT A CUT IN THE FLOOR WHICH EJECTED HIM FROM THE MACHINE. THIS CAUSED A DISLOCATED FINGER AND SOME SCRAPES.
