Showing all 13An employee was operating a water truck, watering down a ramp. The truck started to slide in slick material. Towards the base of the ramp, the equipment tipped over and came to a stop on the roof of the equipment. The employee was able to get out of the cab without assistance and reported pain in their side. They were assessed at a local clinic and diagnosed with broken ribs.
Using Vacuum truck to clean up CKD dust around the Pallet Pan building. A large amount of excess dust moved and struck victim in face.
Two loaded railcars were in the loading bay. The employee was in the first railcar to work the brakes. The two cars did not couple and the cars started moving down the track. The first car stopped causing the second car to hit it from behind. This caused the employee to be knocked into the bar on the end of the railcar bruising his right side.
While checking material flow at the CKD feeder for #11 FM, material blew out of the inspection pipe around his safety glasses and into both eyes.

MAINTENANCE CREW WAS REMOVING 55 GALLON DRUMS OF ROCK WASTE FROM CRUSHER PIT, AS CRANE BOOMED UP, BOOM STRUCK OVERHEAD I BEAM AND EXTENSION JIB ON SIDE OF BOOM THAT WAS HELD IN PLACE BY 5/16" BOLT U-BOLTS BROKE LOOSE AND END OF JIB SWUNG DOWN STRIKING EE ON RIGHT SHOULDER.
PULLED LEFT SHOULDER WHILE TURNING THE MANUAL BRAKE WHEEL ON A RAILCAR. ACCIDENT OCCURRED ON 4/14/03. THE EMPLOYEE FIRST SAW THE DOCTOR ON 4/17/03. THE EMPLOYEE WAS TREATED WITH PHYSICAL THERA PY UNTIL TI WAS DETERMINED SURGERY WAS NEEDED. THIS HAS NOW RESULTED IN A LOST TIME ACCIDENT STARTING 7/11/03.
EMPLOYEE WAS CLEANING #11 CLINKER FEEDER WITH A HOSE FROM THE VACUUM TRUCK. THE HOSE KICKED BACK AND THE WOOD HANDLE STRUCK HIM IN THE CHEST HE RECEIVED CONTUSION, POSSIBLE FRACTURE ON LEFT SI DE RIBS.
WHILE THE EE WAS CHECKING THE IDLERS ON THE EXIT BELT HIS GLOVE WAS CAUGHT BY AN IDLER AND IT PULED HIS HAND IN.

EE WAS PUSHING UP CLINKER PILE WITH 988 LOADER. THE PILE SHIFTED AND LOADER SLIPPED DOWN. EE BRACED HIS KNEE ON THE DASH AND INJURED IT. LEFT KNEE.
