One LNA EE was operating forklift and three iTEC EE's were rigging and positioning new MCC section into the Extruder MCC Room. iTEC EE was positioning dolly when rigging slipped off the forklift forks causing contact between MCC section and iTEC EE. iTEC EE sustained injury to right knee.

IE stood on a belt conveyor to inspect a stone feeder that may have been leaking. IE did not lock out the belt prior to accessing it. Conveyor belt started up and pulled the IE through a 14' height clearance before IE was able to pull the E-stop cable and climb off the belt. Incident caused multiple fractures in employee's back requiring EE to have surgery.

Around 5:40 on 2/3, EE was working in a skidsteer near the south system cage. While on the skidsteer they passed by the dust collector fan DC4030. While backing out of the area the DC fan started blowing out the lime in the skidsteer bucket which got in the EE left eye. EE did not report injury on shift. EE left work at 2:30pm and reported to supervisor at 6:23pm.

EE reports that EE was driving the forklift in the Lower Plant Loading area when EE hit a pothole in the ground. Because of the jolt of the forklift EE felt pain in EE's lower back. At that time, employee chose no medical treatment. On 1/13/2023 the employee visited the doctor where EE was prescribed 6 chiropractic treatments.

Employee was tramming JLG when they engaged a pothole full of water. As the tire dipped the energy transferred to the bucket and caused employee to bounce up and when they came down they twisted their right ankle.

At approximately 5:50 AM, EE entered level 5 of the quarry with the M503 water truck for dust suppression activities. In traversing towards the working face, EE hit a pile of chat used for leveling the quarry floor. The impact launched EE in the cab. Despite being restrained by seat-belt, EE received facial wounds and a fracture of the L3 vertebrae.

At around 8:20, a haul truck overturned on the main haul road. The employee was assisted in exiting the vehicle, taken to the first aid room where EE was evaluated/treated. The employee sustained lacerations/abrasions/contusions from the incident. The employee was taken to the hospital where EE received sutures and was released.

Worker was operating dust truck that hit the road edge and tipped over into the berm. Had mild abrasion to shoulder and a small contusion to the back of the thigh. Complained about elbow pains a few weeks after. On 08/13/19, recommended for physiotherapy as an MRI showed minor tears in tendon around the left elbow, making this a reportable incident.

Employee operating water truck when ABC fire suppressant system unexpectedly discharged exposing employee to small amount of dry chemical suppressant. Employee medically evaluated off-site and diagnosed with lung irritation, the returned to work with restrictions (i.e. no undue exertion and no exposure to "fumes". Note: There was no fire on the vehicle.

Employee and co worker were engaged in replacing sampler near building and large stone conveyor belt. Fork lift operator hit the start/stop button of the conveyor belt with fork lift shutting down the conveyor. Employee struggled with auto sampler when the conveyor belt stopped.

Injured employee was operating Haul Truck. Truck was stationary while being loaded with ore (limestone). Loader Operator accidentally struck bed of Haul Truck with Loader bucket during operations causing Haul Truck to shake violently. Haul Truck Operator reported lower back pain as a result of the incident.

While driving down a haul road, the haul truck struck a soft spot causing the operator's seat to bottom out. When the seat bottomed out, it caused a lower back and neck strain to the operator. The soft spot was removed and filled in with dry material.

Employee was loading a bulk truck. He thought the tank may be empty so he reached outside the handrails and stuck his right hand in a 3"x4" opening to check for material flow. The air-operated cylinder closed the clam shell gate catching his hand between the cylinder rod clevis and the opening.
