Showing all 14Concrete labor was filling fiber form bag with grout using the nozzle of a pump truck. During the process the concrete labor got grout liquid on legs. On 10/19 the concrete labor noticed an itch to their knees, on 10/20 the labor reported a slight redness in the itch area. On 10/24/22 the concrete labor reported the need to see a doctor for redness and a possible chemical burn.
Employee was raising the side gate of a flatbed truck at the same time a forklift was pulling away from the truck. The tire of the forklift struck the gate and the employee's hand was caught between the tire and the truck gate resulting in a laceration to the right index finger that required sutures.
Conditions were wet due to rain. Area was wet,slippery & rocky. While moving a Stacker, EE fell forward tracking the stacker with the remote. As EE fell EE dropped the remote in the mud realizing the remote toggle switches did not return to the neutral. EE moved to find remote, doing this a track of the stacker ran over EE's arch & toes on right foot causing fractures.
The EE was operating the Hemtt fuel truck. It was raining and EE was running behind schedule because of the rain. EE was driving down a steep ramp to fuel a water jack and states that the brakes did not work. Before EE got to the bottom of the ramp EE decided to jump from a moving vehicle. Had EE stayed in the vehicle at the slow rate of speed EE may not of been injured.
While dumping #16-1 Screw Conveyor, an Operator was approximately 10' from the conveyor while a front end loader operator was removing hot rock. As the front end loader approached, a flash occurred which caused hot product to get on both of employee's legs. Employee was taken to first aid where burns were treated, then employee was taken to a local medical facility.
Miner was operating a skidder when the blade on the front of the skidder abruptly fell without warning causing the skidder to stop suddenly. The miner's face, body and left leg contacted the steering wheel and steering column causing his mouth to clench down forcefully causing several chipped teeth, a laceration to the chin requiring 4 sutures and severe bruising to his left leg.
Employee was driving a 3/4 ton pickup down ramp. Contract company was moving 16" pipe across bottom of ramp. Employee's truck hit the pipe causing pain in his right arm/shoulder. Was seen at orthopedics and prescribed physical therapy.
Employee was operating a CAT 740B haul truck in block 8 cut and drove thru a dip in the road while staging up for loading.
After exiting pickup truck, miner stumbled slightly while closing vehicle door causing the door to close on his left hand. The resulting injury was of a nature that required sutures.
Air hose for the brakes on a rail car came apart and the end of the hose struck the employee on the right knee causing a bruise.
Employee was removing a safety can of diesel fuel from the deck of a Genie Telescoping Boom forklift while the operator of the forklift was lowering the boom. The employee's left index finger was pinched between the boom and the handle of the can.
Calcined rock was dumped from #5 calciner hot cyclone at 1:00 pm and cooled with water. After removing half the pile, steam was created during a typical lift when a hot pocket of material contacted water in bucket. Steam and hot material burned employee.
EMPLOYEE WAS OPERATING AN OFF ROAD VEHICLE IN RECLAIM AREA 3. THICK VEGETATION HIT THE GROUND FROM VIEW. VEHICLE FELL INTO A DITCH THROWING OPERATOR AGAINST IGNITION KEY.
