EE was getting out of the 933 skid loader and got right index finger stuck in the door handle that is located on the window. The window of the 933 is the door. Employee was opening the door and getting up at the same time. Finger got caught in the latch and twisted finger on handle before letting go. Door is hard to open.

Rock rolled from truck bed, as truck was being loaded, landing on fire extinguisher located on front deck. The fire extinguisher was activated and ABC extinguishing agent got into driver's eyes

980 Loader Operator was instructed to move rock out of roadway at dump R2. While dumping rock operator hit a pipe in the berm and was jarred by the impact.

Normal field operating conditions, operator's lack of awarness resulted in laceration of right Tricep.

Employee driving-began to cough, lost control of vehicle ran off side of road.

Removing tire from machine. Extremely tight bead on wheel due to no lubrication, resulting in ram slipping off pressure point and striking EE in the face.

LOADING CONEX W/PALLETS OF ANODES, PINCHED FINGER IN FRAMING AROUND ANODES. REQUIRED A DR.'S VISIT, NAIL WAS REMOVED & (6) STITCHES APPLIED.

EE WAS DIRECTING THE 939C OPERATOR WHILE HE WAS PULLING A PIECE OF HDPE PIPE. THE CHOKER SLIPPED OFF THE PIPE. WHEN THE PIPE MOVED, IT STRUCK EE'S LEGS AND KNOCKED HIM TO THE GROUND, SCRAPING HIS LEFT CHEEK AND FRACTURING HIS NOSE.

1.INJURED WAS TRAVELING WITH THE BUCKET IN THE RAISED POSITION.2.DENSITY OF THE PIECE THAT WAS IN THE BUCKET. 3. PIECE LANDED ON THE CONCRETE TOP AND LINER PLATE BOLT. 4. SIZE OF THE LINER PLATE BOLT. 5. LINER PLATE BOLT FLEW INTO THE BOBCAT CAB.

INCIDENT NOT REPORTED UNTIL 6/28/04 (AFTER EE HAD BEEN TERMINATED). EE HAD NOT SOUGHT MEDICAL TREATMENT AT THAT TIME. HE INFORMED CO 7/12/04 THAT HE IS GOING TO SEEK TREATMENT FOR SORENESS IN HIS SHOULDER. EE ALLEGES WHILE OPERATING A JLG MANLIFT, HE CAUGHT THE LIFT WHILE RAISING IT. AS IT BROKE FREE THE LIFT JERKED & HE WAS JOSTLED AROUND. LATER HE FELT SORENESS IN SHOULDER.
