Employee stood up in the skid steer to lock on the bucket and the boom raised and caught his arm between the boom and the cab resulting in a laceration to his arm.

EE climbed up into the front-end loader, his hard hat fell off, EE bent over to pick it up and struck his head on railing.

EE alleges that while operating loader lower back started to tighten up and get sore.

While installing a tail roller in a stacker conveyor the tail roller rolled back and smashed his finger.

Employee indicates that when woke up in the morning he was bearly able to get out of bed. He relates the pain to operating an old loader.

EE reported that over the course of a day and half of running the loader from the lower quarry to the north dirt pile he began to feel pain.

Employee indicates that twisting his neck around while back the loader repetively caused his injury.

Employee drove a loader in 3rd gear into the sand wall to scoop up a bucket full of material for the plant and felt a sharp pain in her back.

As employee scraped the floor of the quarry with the loader, the bucket dug deep into the floor resulting in the loader stopping suddenly and he struck the windshield.

While watching the material come off the conveyor and resting his hand on the rock box, employees hand was struck by a chunk of material, causing the injury.

During normal haul truck operation employee experienced upper back strain. The cause is undetermined.

EE REPORTED BACK PAIN BELIEVE TO BE CAUSED BY THE LOADER SEAT.

EE INDICATES THAT THE SEAT IN THE LOADER WAS GETTING BAD AND RESULTED IN GRADUAL ON SET OF BACK PAIN. EE REPORTED INJURY ON THE 13TH AND QUIT ON THE 14TH.

EE STARTED HIS FIRST DAY OPERATING LOADER FEEDING THE PLANT. HE BEGAN TO EXPERIENCE BACK PAIN THE SAME DAY. HE WAS RELEASED TO RETURN TO WORK ON JUNE 23RD, 2003 AND QUIT THE SAME DAY.

WHEN MAKING REPAIR ON THE CRUSHER EE GRABBED THE V-BELTS TO KEEP HIS BALANCE AND THE BELT ROLLED AND PULLED HIS HAND BETWEEN THE BELT AND CHEV OR V-PULLEY.

THE BANK OF SAND AND GRAVEL SLOUGHED OFF AND COVERED THE FRONT END OF THE LOADER. IT SMASHED THEWINDSHIELD RESULTING IN INJURY TO THE OPERATORS HAND AND ANKLE.

EE WAS OPERATING 988 FEED LOADER @ CRUSHING PLANT (WED-FRI). WHEN HE WOKE UP OS SATURDAY MORNING, HE FELT PAIN INHIS LOWER BACK. BEGAN TREATMENT ON SUNDAY.

EE WAS OPERATING HAUL TRUCK WHEN HE STARTED HAVING PROBLEMS WITH HIS BACK DUE TO THE ROUGH RIDE.

EE'S BACK STARTED GETTING STIFF FROM OEPRATING LOADER EARLY IN THE SHIFT. HE CONTACTED HIS CHIROPRACTOR FOR TREATMENT.

WHILE OPERATING THE LOADER THE CUCKET HIT AND JUMPED OBER A BOULDER CAUSING HIS NECK TO SNAP.

WHILE EXTENDING THE WHEELS ON THE STACKING CONVEYOR, EE INSERTED THE PIN WHICH RESULTED IN PINCHING HIS THUMB.

EE WAS BOUNCING IN HIS LOADER OVER ROUGH TERRAIN MUSH OF THE DAY, CAUSING PAIN IN BACK & LEGS.

EMPLOYEE REPORTED TO HR THAT HE HAD BACK PAIN THAT WAS INTERUPTING HIS SLEEP AND THAT THE PAIN MAY BE CAUSED BY THE LOADER. INITIALLY THIS WAS THOUGHT TO BE A FLARE UP FROM A PREVIOUS CLAIM. RECENTLY IT WAS DECIDED THAT A NEW INJURY DATE WOULD BE ASSIGNED.

EE WAS USING A LOADER TO HAUL MATERIAL TO THE BREAKER-HE STRUCK A TOE ON QUARRY FLOOR WITH LOADER BUCKET AND IT THREW HIS HEAD TO THE LEFT AND THEN HE HIT ANOTHER TOE IN THE WALL WHICH CAUSED HIS HEAD TO FLY FORWARD.

WHILE OPERATING FRONT END LOADER STOCKPILING, HIS LOADER TIRE (FRONT) HIT UP ON THE SAND PILE CAUSING HIM TO TWIST IN THE SEAT, WRENCHING HIS BACK.

STEERING FAILED IN HAUL TRK WHILE TRAVELING DOWN HILL THE TRK VEERED TO THE LT PLUNGING INTOBERM TRK BOX OVERTURNED
