Operator was struck in the hand while releasing the handle on a ratchet strap. Impact from the handle fractured Operator's thumb.

Operator was lifting up railing on cone crusher and strained lower back.

Operator was using a pry-bar to reposition the top cross conveyor when he felt a pop in his right shoulder.

While lifting dredge pipe employee felt pain in lower abdomen and reported it as report only. He sought treatment on 10-28-10.

Employee indicates he was walking around the plant with safety glasses on; an object flew into his left eye.

Employee had the sample catcher on her right shoulder. As she bent over to pick up the sample pail the catcher fell and struck her right shoulder.

Employee was changing out ware plates in the horizontal impact crusher when he turned and felt a pain in his lower back.

Employee was replacing teeth on a 330 Volvo front-end loader bucket. Kyle attempted to drive the pin out of the tooth with a mallet and struck his thumb as a result. Kyle began to loose time on 6/29/04.

WHEN MAKING REPAIR ON THE CRUSHER EE GRABBED THE V-BELTS TO KEEP HIS BALANCE AND THE BELT ROLLED AND PULLED HIS HAND BETWEEN THE BELT AND CHEV OR V-PULLEY.

EE WAS OPERATING HAUL TRUCK WHEN HE STARTED HAVING PROBLEMS WITH HIS BACK DUE TO THE ROUGH RIDE.

THE WELDER PLACED THE HAND GRINDER THAT HE HAD BEEN USING ON THE FENDER OF THE TRAILER THAT HE WAS WORKING ON. APPARENTLY SOMETHING CAME OFF THE GRINDING WHEEL AND STRUCK HIS EYE FROM BEHIND T HE GLASSES.

WHILE CLEANING AROUND PLANT, EE PICKED UP A LARGE WOODEN BLOCK THAT SLIPPED FROM HIS HAND & STRUCK TOE

STEERING FAILED IN HAUL TRK WHILE TRAVELING DOWN HILL THE TRK VEERED TO THE LT PLUNGING INTOBERM TRK BOX OVERTURNED
