EE alleges that while operating loader lower back started to tighten up and get sore.

While climbing down ladder of 988 cat loader, employee slipped and grabbed ladder with left arm to hold himself when felt/heard pops.

Employee was using a maul to hit a shank cover on a 988 bucket. A very tiny metal fraqment broke off the maul head and struck employees lower lip apparently embedding in the lower lip.

While attempting to unplug the Jaw with a wedge, the wedge got snagged and pinched his foot on the rail.

EE WAS USING AN AIR HAMMER TO REMOVE MUD & CLAY OUT OF ROTO-LIFT ON THE ROLL PLANT WHEN HIS SHOULDER BECAME SORE-PAIN INCREASED OVERTIME, SO HE IS SEEKING TREATMENT. SOUGHT MEDICAL TREATMENT IN MAY.

MINER WAS WORKING ON THE JAW CRUSHER TIGHTENING THE TOGGLE PLATE. AFTERWARD HE STARTED TO HAVE PAIN IN HIS SHOULDER. THE MINER HAS SOUGHT MED TREATMENT.

EE SHOULDER BECAME PAINFUL WHILE ADJUSTING THE JAW OF THE BREAKER, PUSHING UP WITH A WRENCH.
