Conveyor belt stopped on sand line. Operator reached past guard to pull belt trying to free operation. Operator did not follow lockout/tagout procedure. Operator got right hand caught between conveyor belt and head pulley. Right pinky received 8 stitches and ring finger has fracture on bone at tip of finger.

Employee ran into pot hole while driving forklift, resulting in cervical strain. Employee returned to full duty for full shift on the following day.

Operator was operating moffett forklift when they extended the forks to pickup a load. Employee then proceed to back up with the load on the extended forks off the trailer, the weight caused the forklift to tip forward due to the extended forks. Operator hit head and neck when machine tipped forward.

Employee was removing sand bag from turn table when another bag transitioned from sand line conveyor and fell on wrist

An employee was driving a forklift with a couple of pallets and was not able to see in front of him while another employee walked in front of him and caught his right foot with the pallets. Employee was taken to emergency room to x-ray foot and found no fractures but very bruised up. The nurse said it was a contusion.

Employee tried to remove debris from conveyor without shutting off line. Glove became caught in roller causing a portion of employee's left thumb to be cut off.

EE was being lifted on a platform with a forklift to loosen cement stuck in Portland screw. When forklift driver lowered the lift, the platform jerked and the EE lost his balance and fell to his knees. Has pain in his knees

Employee used foot to move a bag on a conveyor belt without stopping the conveyor. Shoe lace was caught in the conveyor. Result of the accident was a scrape and bruising to the top of employee's right foot.

Employee was in the process of hauling (1) pipe in bucket. While traveling down driveway, employee passed out. The Komatsu drifted off roadway, over a berm, and tipped over onto left side. Employee was wearing seatbelt. With the help of others, employee climbed out of Komatsu.

Mis communication with Forklift driver. Forklift driver set pallet down on wrap machine platform-did not realize injured employee s hand was still there.

EE placed hand against conveyor belts rotation, his hand became trapped. left hand was injured, skin was pulled back exposing bone and was bleeding.

EE was sweeping floor when forklift backedup and bumped him left nakle was twisted as a result.

Forklift operator was backing out of isle and turned as he cleared the edge of isle. The rear wheel of the lift went over the pedestrian's foot crushing the foot. At this time there is no sign of broken bones.

EE WAS GREASING A BEARING ON THE PALLETIZER. HE WENT TO CLIMB DOWN & CONVEYOR CAUGHT HIS GLOVE WITH HAND & PULLED IT INTO THE CONVEYOR. BRUISED HAND AND CHIPPED BONE IN LITTLE FINGER.

DRIVER HELPING TO PUSH PALLET OFF FORKLIFT AT CUSTOMER'S SITE. FORKLIFT DRIVER PUSHED THRU PALLET & HIT OUT DRIVER'S BIG TOE, BREAKING TIP OF TOE.
