Tried to remove piece of plastic from bag sealer while still running. Lost tip of right index finger.

EE was being lifted on a platform with a forklift to loosen cement stuck in Portland screw. When forklift driver lowered the lift, the platform jerked and the EE lost his balance and fell to his knees. Has pain in his knees

Mis communication with Forklift driver. Forklift driver set pallet down on wrap machine platform-did not realize injured employee s hand was still there.

EE placed hand against conveyor belts rotation, his hand became trapped. left hand was injured, skin was pulled back exposing bone and was bleeding.

Employee was putting and pulling belts on the dry elevator. When pulling, his hand got caught and his finger was broken.

He was on top of the Q Line reclaim hopper next to the weigh pod. He was putting the top back on the weight pod, tightening the belts. The wrench slipped. He lost balance & fell backwards. Pulled muscle in middle/lower left side of back.

Busted bag stuck between conveyors. Employee tried to free it - hand got caught. Took off tips of middle and rings fingers, skin across top of hand and two fingers was pulled off. Tips reattached and skin graft to repair hand.

EE WAS GREASING A BEARING ON THE PALLETIZER. HE WENT TO CLIMB DOWN & CONVEYOR CAUGHT HIS GLOVE WITH HAND & PULLED IT INTO THE CONVEYOR. BRUISED HAND AND CHIPPED BONE IN LITTLE FINGER.

EMPLOYEE WAS WATCHING R-LINE AND TRIED TO STRAIGHTEN BAG OF CEMENT. BAG BROKE AND FLEW INTO HIS EYES CAUSING A CHEMICAL BURN. SLIGHT SCRATCH TO RIGHT EYE.
