Plant was shutdown to address problem on #2 screen feed belt. The tail pulley was removed and loaded into the skid steer bucket to be moved out of the way. While backing and turning to the right an employee walked into a blind spot causing the skid steer bucket to make contact with EE's left knee.

The employee was operating a wheel loader to move Type III stone to the stockpile yard. While digging into the pile, the loader jolted as the bucket came to a stop, and the operator felt pain in neck and upper back.

Operator was scooping GAB from the pile when the top of the pile sloughed off and filled the bucket, causing the loader to lose balance and tip forward. The operator quickly dropped the bucket, causing the rear of the loader to fall back to the ground and jar the driver. The driver reported experiencing soreness in their back after the event.

Employee reported intermittent left knee soreness at the end of shift, but couldn't think of any incident or event that could have caused it and didn't think that it was work related. The employee sought medical care when the issue didn't resolve, and was advised that it could be related to repetitive pushing of the clutch pedal on the site's front end loader.

The injured employee was adjusting the belt scraper on C-6 conveyor. The belt was still in motion as EE attempted to adjust the scraper and this caused the spud wrench to forcefully be spun back causing it to contact EE's finger against a rail. The employee was taken to the ER and given stitches and a splint.

In process of removing cheek plate from jaw that was binded (jammed) up for removal, attached crane to cheek plate and applied pressure and cheek plate popped out and swung 5 feet and caught welder fabricator in face and knocked EE backwards. Worker got bloody nose and 5 stitches. Went to doctors and told to take day off.

While removing material from the loading face, the pit loader came to a sudden stop, "jolting" the machine. Later in the shift, employee reported some discomfort between the shoulder blades. First aid/self care measures were employed. A clinic visit was required on 9/23. At that time, the employee's work duties were restricted from operating equipment due to medical precautions.

The employee was operating an articulated truck. Employee stated employee fell asleep. The tire tracks indicate the truck rode up the left side embankment causing the cab to turn over on the right side. Employee received a concussion without loss of consciousness and a laceration to the right ear without foreign body.

Employee loading trucks in pit with 990H stated a 6'x2' rock slid appr 13' down muckpile and struck left front corner of the bucket and felt a pain in neck. Clinic placed employee on restricted duty on 8/14/19, currently assigned to operate smaller loader

Employee was operating pit loader and digging in muck pile to load a haul truck, when the bucket of the loader hit a rock toe and the loader came to a sudden stop. This resulted in a jarring motion and the employee felt a pain in his right leg. This originally did not require medical treatment, but a subsequent doctor's visit resulted in work restrictions and physical therapy.

Rubber tired loader operator was running the machine with malfunctioning electrical magnetic locks on the hoist and tilt controls. In the manual mode more effort is required and the operator strained his right arm.

A large rock slid from the top of the muck pile and before the operator could back out of the way the rock slammed the bucket of the Caterpillar 988B pit loader. This caused the employee to lurch forward hurting his neck in the process. He was diagnosed with a neck sprain.

WHILE EE WAS DRIVING HAUL TRUCK INTO THE PIT AREA, HE WENT OVER HIGHWALL IN PLANNED DRILLING AREA, CAUSING TRUCK TO FLIP OVER. DRIVER HAD A MINOR CUT TO HIS LEFT LEG, SUSTAINED WHEN HE UNBUCK LED SEATBELT WHILE HANGING UPSIDE DOWN IN CAB, AND FELL ONTO DASH AREA.
