Employee was operating haul truck and cut his right middle finger on a sharp object in the cab of the truck.
Employee was trying to obtain a sample from the clinker conveyor belt. While getting the sample, the sampling scoop got stuck between the guard and the conveyor. The motion of the conveyor pulled the scoop upwards smashing the employee's finger against the guard.
On 9/5/2006 employee stuck his finger in an air condition unit fan blade located inside the cab of a haul truck and received a minor laceration to his finger. On 9/13/06 the finger was not healing properly and he was taken to the doctor for treatment. The wound was cleaned and he was given antibiotics to prevent an infection.
He walked down the first set of stairs from 992 G cab level to the ballast block level without any incident. He faced the engine compartment, held on to both handrails and using his right foot, took one step down. The right side support bracket and bolts were broken. As the step dropped, employee knocked his right shin against the walkway edge.
Employee was operating an 80-foot High Reach Man Basket. The employee decided to travel across a shallow swale used to drain water. The front tires of the machine dropped into the swale, which catapulted the basket upward. The employee was thrown into the air then yanked back into the basket because he was wearing fall protection. He fractured his right leg.
Employee injured by falling stone from overflow of surge bin while installing chute above 4 1/4 chute. (Belt was operating. Multiple stones fell, but EE was not buried.)
EE WAS JARRED INSIDE CAB WHEN A BOULDER ROLLED OFF MUCK PILE AND STRUCK LOADER.
EE was pulling a stuck vehicle free from mud and the factory pull hook broke free from the truck which was stuck and the pin struck the 980G loader through the rear window hitting the loader seat from behind, which caused an injury to his right shoulder.
WHILE REMOVING A FEED PLATE FROM THE CRUSHER WITH A CRANE, EE'S HAND GOT PINCHED BETWEEN THE FEED PLATE AND THE RIM OF THE CRUSHER.
EE WAS BACK DRAGGING ROAD ADJACENT TO MINE PROPERTY WHEN HE STRUCK A TREE. LATER THE EE COMPLAINED OF CHEST PAINS THEREFORE, SUSTAINING A STRAINED CHEST MUSCLE.
EE WENT TO REMOVE AN EMPTY BAG FROM THE STOPPED CONVEYOR LINE WHEN THE LINE STARTED AUTOMATICALLY HE CAUGHT HIS RIGHT THUMB IN THE CHAIN CONVEYOR. EE FAILED TO HIT THE E STOP PRIOR TO REACHING FOR THE EMPTY BAG. HE DID NOT HIT THE E STOP SINCE THE CONVEYOR WASNT RUNNING.
EE WAS WASHING OUT TAIL SECTION OF CONVEYOR WITH WATER HOSE WHEN A ROCK FELL FROM OVERHEAD CONVEYOR AND STRUCK EE IN HARD HAT.
AN EMPTY BAG GOT ONTO THE CONVEYOR. THE EMPLOYEE WAS REMOVING IT WHEN HIS RIGHT THUMB GOT CAUGHT IN THE MESH CONVEYOR. HE THEN PULLED HIS THUMB FROM THE CONVEYOR, ADDING TO THE INJURIES.
TECHNICIAN HAD TO UTILIZE MANLIFT TO PERFORM MAINENANCE ON SCALE READOUT. THEY FINISHED & MOVED THE MANLIFT TO AN OUTSIDE AREA. WHEN THEY ATTEMPTED TO STOP, THE UNIT SHUT OFF ABRUPTLY WHICH FO RCED THEM ONTO THE SAFETY RAIL OF THE MACHINE & BRUISED THEIR RIGHT SIDE.
WHILE MOVING VEHICLE, EE WAS HIT BY ANOTHER VEHICLE MAKING LEFT TURN IN FRONT OF HIM.
LOADER OPERATOR SUSTAINED LEFT SHOULDER STRAIN WHILE LOADING TYPE ONE RIP RAP TO A CUSTOMER TRUCK. HE STRUCK A "TOE" IN THE STOCKPILE (PIT) WHILE ACCELERATING TO HAVE POWER TO LOAD.
EMPLOYEE WAS DRIVING HIS TRUCK AND HIT A SPEED BUMP, INJURING HIS STOMACH WALL.
ON 11-29-01 BETWEEN 12-2PM EE WHO IS THE YARD LOADER FOR A 980F, WAS ABOUT TO LOAD A TRUCK (CONTRACTOR) WHEN HE DUG HIS BLADE OR BUCKET ANDSTRUCK A ROCK IN THE STOCKPILE. EE REPORTED HE WAS "JARRED" INSIDE HIS CAB AND THUS HURT HIS NECK (STRAINED). HE WAS LOADING #1 RIP RAP.
WHILE DRIVING HAUL TRUCK HE HIT A POT HOLE AND HURT LOWER BACK.
WHILE A PASSENGER IN VEHICLE, DRIVER TRIED TO AVOID MATERIALS NEXT TO TRACK & HIT A STANDING TRAIN. HE FELT PAIN IN UPPER LEFT LEG & KNEE.
EMPLOYEE WAS DRIVING A HAUL TRUCK. HE SAYS HE WAS REACHING INTO HIS LUNCHBOX FOR A SODA WHEN THE NEXT THING HE KNEW, HE WAS OFF THE HAUL ROAD. HE RODE THE BERM FOR 20 FT AND THEN ENDED UP IN T
EMPLOYEE PINCHED NERVE ON NECK WHILE WORKING ON THE LOADER.
EE WAS PUSHING DREDGE WITH THE BOAT WHILE THE BOAT WAS IN GEAR, THE BOAT STRUCK THE LADDER PORTION OF THE DREDGE PINNING HIS LEG, CAUSING A LACERATION TO HIS LEFT LEG.
EMPLOYEE WAS REACHING TO TURN OFF AC UNIT IN CAT LOADER WHEN HE SLIPPED AND HIS RIGHT INDEX FINGER WAS LACERATED BY AC FAN BLADE.
WHILE LOADING TRUCKS WITH FRONT END LOADER EE JARRED IN SEAT AND STRAINED HIS BACK.
EE STRAINED LEFT HOULDER WHILE OPERATING A FRONT END LOADER, LOADING HAUL TRUCKS WITH EXCAVATED LIMESTONE IN THE PIT.
