Employee was operating a John Deere 300D end-dump haul truck while performing dirt stripping activities. Employee was pulling away from dump-site when a hydraulic line supplying pressure to the bed ruptured causing the bed to slam on the frame of the tractor. Employee didn't wish to seek medical treatment and completed tasks for the entire shift.

Showing all 3On 8/30/10 employee was greasing the bottom right pin of a 988G when the operator tilted the bucket back rolling it onto the right part of the right foot smashing his foot. This crushed the two right bones going to the two right toes just behind his toes.
On 4/29/10 EE reported that back on 3/3/10 he was sweeping rocks off of the pit floor with the 988G loader he was operating when he caught the bucket on something solid causing a sudden stop of the loader. When this happened he said it felt like it ripped his insides. The accident was not reported to the supervisor until 4/29/10.
