The ee was struck in the face with a 4" water line. The empty discharge line was struck by the shuttle car and in the turn hit the ee in the face. The ee was dazed but stated that he was ok and completed his shift. Later the ee went to the hospital and it was determined he had three fractures to his jaw.

A utility man was injured when the rubber tired vehicle he was riding was struck by a second vehicle. The ee (passenger) was exiting the mine at the end of his shift when the second vehicle which had been parked in a crosscut suddenly and unexpectedly backed out and into the path of travel.

A scoop operator received lacerations to his middle and ring fingers on his left hand when he was moving a shuttle car and the shuttle car dropped off of a small ledge in the mine floor. He had positioned his left hand outside the shuttle car compartment and his fingers were smashed between the operators deck and the mine floor.

The SC operator was making a right turn into the #3 heading when ee over steered SC & struck a row of timbers. The ee alleged that he moved over & raised up in his seat to avoid being hit by the timbers & struck his head on the mine roof. The ee had 12" of clearance over his hard hat. This report is being submitted under protest. The ee resigned on 5/7/12.(attachment to follow)

A scoop op.was traveling in the #3 entry. The life line for the secondary escape way was on the right side of the entry over the scoop op's compartment. The life line became hooked on the deck of the scoop and as the operator made a right turn the life line and spool became tangled and entered the operator's compartment and struck the op. in the upper lip resulting in a laceration.

The 3rd shift utility crew had locked-out the R-2 belt & were removing slack from the belt. The day shift section crew stopped at the R-2 drive & found the belt was locked-out. The day shift electrician thought the lock on the cathead was a lock he had placed, and had lost the key to the lock. The lock was knocked off & the belt energized resulting in the incident.

The tram stopped at the #4 belt drive to give the belt man a ride to the #3 drive. As the tram left the #4 drive the hood of the belt man's jacket caught on a roof bolt & he was pulled over the back of the operator's compartment of the tram. This injury resulted in a laceration requiring 16 stitches to the employee's chin from his jacket zipper.

The 2nd shift production crew was exiting the mine on a Mac 8 rubber tired mantrip near break 18 when a slate bar that was located on the mantrip caught the mine floor resulting in the bar being impaled into the upper leg of an ee that was seated on the left side of the man trip. The ee was taken to the hospital were surgery was performed to remove the slate bar.

The ee was positioned in the operator's compartment with the driver of manbus. The bus traveled near break #36 the mine height changed from 72" to 54" quickly resulting in the belt man's head contacting the mine roof due to not being positioned low enough to provide vertical clearance. The belt man received an abrasion to the forehead.

The employee was using a scoop to transport his tool box to a different location. The incident has been investigated and the employee has been interviewed but the cause is unknown. The ee was taken to the hospital for examination and was released to full duty but did not work. The mine was notified on 7/29/11 that the employee was being treated for a head injury.

The employee was riding the mantrip with the section crew at the end of his shift. As the mantrip traveled through a ventilation curtain, the miner trainee raised his left leg to hold up the curtain. The employee's leg was caught on the curtain board resulting in a hyperextension of his knee.

The employee was a passenger on a little mac personnel carrier when his belt-wearable SCSR contacted a fallen rib and pushed his SCSR into his right side resulting in bruising of his side.

The crew was on the manbus and stopped to raise an air-lock door. The employees riding on the inby end of the manbus changed from laid down position to riding on their knees. As the bus traveled through the door one employee caught his hard hat on the door frame, pushing his hard hat onto his safety glasses, resulting in a laceration to his nose and under his right eye.

The employee was riding a rubber tired personnel carrier in a laid-back position, due to the mining height. When he exited the personnel carrier his right foot was lodged under the personnel carrier holding his foot stationary while his upper body turned. This created a twisting motion at the knee, resulting in a knee injury. Lost time began on 8-6-10.

The injured, a contractor employee, & a company employee finished making a belt splice. After loading the belt splice equipment on the personnel carrier, they boarded the ride to go to the section. The vehicle started faster than they anticipated & the vehicle ran under the belt line. The contractor emp. struck the side of his face on a belt roller hanger.

The employee was bounced in the shuttle car while traveling to the feeder. He stopped the shuttle car to reposition himself. He grasped the bottom of the operator's deck and the shuttle car drifted backward catching and rolling the tips of his fingers between the deck and the mine floor.

The emp. was operating a track manbus traveling to the #4 Belt Drive to begin cleaning belt. Approaching the track switch at the #4 belt drive, another emp. was at the switch to throw it when the injured went through it. The injured was looking & talking to others as he was moving through the switch & caught his head against the roof & jerking his head backward.

High voltage cable was being pulled back and loaded into a rubber tired flat car. The car was being pulled by a scoop and as the car was moved forward, the EE had his left foot in front of the wheel and the wheel of the car ran over his foot.

Emp. took a load of rock dust in with the track mounted rock dusting machine attach. to rock dust belt lines. He went to get more rock dust arriving at Bratt. No. 65 on No. 2 belt line, one of the steel tracks broke-and one of the ends raised up & caught bottom of the rock dust mach. & the emp. was forced into the side of the operator's compartment of the tram.

The mantrip had stopped to re-hang a water line that was across the track. The op. got off the vehicle to help and hit the dump valve for the parking brake. The dump valve didn't fully relieve & in the process began rolling toward the employees. The injured jumped to the side and was caught between some stacked rock dust & the mantrip vehicle & was rolled beside the vehicle.

The production crew was leaving the section at the end of the shift. As they were traveling to the end of the track in the bucket of a battery powered scoop, the employee apparently lost his balance and it is believed he struck his head on the cap lamp of the employee next to him. The blow cut his ear.

The injured and co-worker loaded one end of the track vehicle with rock dust and placed a mandoor on top of the dust. The injured unloaded rock dust at the No. 2 - 5 belt drives and was proceeding to the #6 belt drive when the mandoor caught the roof and was dragged over the top of the track vehicle and struck him on the left arm.

The section was being moved around and a new belt was being installed. The belt tail piece had been disconnected and was to be moved to the belt being installed. A scoop was hooked to the tail piece to move it. The EE did not move back far enough. One of the legs of the tail piece fouled on an offset in the mine floor. It swung around and the back of the tail piece hit the EE.

The employee was taking a load of rails in on the rail car - the car derailed. Using a hydraulic jack, he jacked the car up from the middle of the inby end. After doing this, the car still had to be jacked up some in order for the wheels to clear the track. EE had his hand on the car as he was jacking the car up, the car creeled onto the hydraulic jack catching his left hand.

The employee was checking fire outlets along the No. T-5 belt line. He was riding a 3-wheeled personnel carrier. The personnel carrier hit a rock or dip & jerked the steering wheel out of his hand. The area where the accident occurred was sloping toward the belt line & the carrier slid under the belt which was not running. EE struck his shoulder on the belt rail.

The employee was taking a loaded shuttle car to the feeder. As he passed his cable anchor he thought the cable was coming in the deck on him. He threw his arm up & stopped the shuttle car. The cable then took up catching his elbow & pulling his arm upward & backward into the canopy.

At the start of the shift, the EE was operating track mounted vehicle to his section. As the vehicle went through the 2nd set of airlock doors, an unsecured lid over the traction motor compartment slid out of place and contacted the right support of the airlock door. The lid sprung foward hitting EE and pushed his face into the cover over the contactor compartment of vehicle.

Mechanics were installing a canopy on a shuttle car. The canopy was laying upside down; in order to orient it properly they had the scoop operator bring the scoop to it. The mechanic hooked a chain to the canopy & to the scoop's ram jack to pull the canopy up in the scoop bucket. The canopy caught on the lip of the bucket, swung around & the corner landed on the injured's foot.

AS HE WAS TRAVELING THE TRACK ENTRY AND STARTED DOWN AN INCLINE, APPROX 3-4 BREAKS INBY THE DRIFT MOUTH A SLATE BAR THAT HAD BEEN LEFT ON TOP OF THE PERSONNEL CARRIER SLID FORWARD, STRUCK A PROP SET ALONG THE BELT. THE BAR HIT EE'S LEFT ARM BREAKING IT & CUT UPPER LIP REQUIRING 7 STITCHES.

AS THE PERSONNEL CARRIER WAS BEING BACKED OUT OF THE BREAK, THE EE TURNED HIS HEAD TO THE RIGHT TO LOOK BACK. HE STATED ABOUT THIS TIME, THE PERSONNEL CARRIER HIT THE COAL RIB & HIS NECK STARTED HURTING.

EMPLOYEE WAS OPERATING A SHUTTLE CAR AND HE WAS TRAVELING TOWARD THE DUMP, THE SHUTTLE CAR RAN OVER A CRUB BLOCK ON THE OFFSIDE OF THE OPERATOR. THE SHUTTLE CAR BOUNCED AND THE EMPLOYEE CAME D OWN ON EITHER THE BRAKE PEDAL OR THE SIDE DECK OF THE SHUTTLE CAR. THE CRIB BLOCK WAS LEFT BESICE THE ROADWAY WHEN THE HEAD OF THE MINER WAS CHANGED OUT.
