Accident type, without injuries

A roof fall was found in No.3 entry in old 1st left section off mains at SS #157. Fall measured 40'l x 20'w x 5'-6'thick. Area was driven in 1994 and had been bolted with 36"" conventional roof bolts.

A roof fall was discovered approx 1200'-1500' from the faces of the 001-0 section. The fall occurred two breaks inby survey staion No.2874 and measured approx 70'-X 20' wide X 2' - 4' thick. The area had been bolted with 36"" fully grouted resin roof bolts and some cribs had been installed in the area.

Struck against stationary object

At the start of the shift, the EE was operating track mounted vehicle to his section. As the vehicle went through the 2nd set of airlock doors, an unsecured lid over the traction motor compartment slid out of place and contacted the right support of the airlock door. The lid sprung foward hitting EE and pushed his face into the cover over the contactor compartment of vehicle.

A roof fall was discovered one break inby survey station #6970 in the 16th developement during a weekley examination of the area. The fall measured approx. 20' long x 20' wide by 4'- 5' thick . The roof was composed of laminated sandy shale and had been supported with 36' fully grouted resin bolts.

A ROOF FALL OCCURRED IN THE NO.4 ENTRY (BELT) ENTRY JUST INBY SURVEY STATION NO 2863 AND APPROXIMATELY 1400 FEET FROM THE ACTIVE 001-0 (P"") SECTION. THE FALL MEASURED APPROXIMATELY 45' LONG X 20' WIDE X 5'-6' THICK AND WAS COMPOSED OF LAMINATED SANDY SHALE. THE FALL CAUGHT THE 001-0 SECTION BELT LINE IN THE AREA OF THE FALL.

Jacked JB Long three-wheeler up. He was letting jack back down; bar slipped out of his hand and came up, catching him under his chin cutting him.

A roof fall occurred in the NO. 2 return entry in the NO. 2 heading approximately 500' outby the face of the 001-0 section. The fall, in a slip form measured approximately 25' long x 16' - 18' wide x 0 - 5' thick. The rock fell above the anchorage zone of the roof bolts installed in the area.

EMPLOYEE WAS TAKING SUPPLIES UNDERGROUND IN A UNIHAULER. APPROACHING THE VENTILATION DOORS HE GOT OUT AND OPENED THE 1ST SET OF AIRLOCK DOORS. HE MOVED THE UNIHAULER BETWEEN THE DOORS, GOT OFF TO CLOSE THE OUTBY DOORS. AS HE WAS TURNING TO CLOSE THE RIGHT SIDE DOOR, AN AIR CURRENT CAUGHT THE RIGHT SIDE DOOR CAUSING IT TO SHUT AND CAUGHT HIM BETWEEN THE TWO DOORS

The ee was installing roof bolts in the 1st cut that had been taken out of the No. 3 left crosscut. As the ee began walking to the head of the bolter from the tram station, he stepped upon a step up in the mine floor. As he stepped upon the 4"" step up his foot slipped off and the ee twisted and hyperextended his knee.

EMPLOYEE WAS USING A HAND HELD HYDRAULIC DRILL DRILLING A HOLE FOR AN ANCHOR PIN TO ANCHOR TAILPIECE, WHEN THE GASKET IN THE TOP OF THE DRILL BLEW OUT CAUSING HYDRAULIC OIL TO HIT HIM IN THE ARM.

AS THE CONTINUOUS MINER ON THE SECTION WAS CHANGING PLACES, THE EE WAS HELPING MOVE THE MINER CABLE OUT OF THE ROADWAY. AS EE WAS PULLING CABLE, HE STRAIGHTENED UP & FELT A POP IN HIS BACK.
