AFTER HE LOADED THE TRUCK IT STALLED. BY THE TIME HE HAD THE TRUCK RE-STARTED, HE WAS ALREADY IN THE CURVE WHERE THE ACCIDENT TOOK PLACE. HE DID NOT HAVE ENOUGH TIME TO REGAIN CONTROL OF THE TRUCK.

AS EMPLOYEE WAS BACKING TRUCK INTO POSITION TO DUM P FILL MATERIAL FOR REDAURATION ACTIVITIES, THE ROADWAY BROKE AWAY CAUSING THE TRUCK TO TURN OVER. THIS CAUSED A LACERATED UPPER LIP AND LACE RATED LEFT ARM WHICH RECEIVED FIVE STITCHES.
EMPLOYEE WAS HAULING TO FILL AREA WHEN HE HIT A DIP IN THE ROAD THAT CAUSED HIS HEAD TO STRIKE THE TRUCK CAB.
EE PICKED UPA BUCKET OF SHOT ROCK. A LARGE ROCK SLIPPED OUT OF THE BUCKET CAUSING THE LOADER TO BOUNCE QUITE SEVERELY. HE WAS SLUNG INTO THE SIDE OF THE CAB COMPARTMENT CAUSING LACERATIONSA TO THE HEAD. DR.'S APPLIED 8 STAPLES TO CLOSE WOUND.
EE WAS HAULING SPOIL TO FILL AREA. TRUCK SLID SIDEWAYS AS HE APPROACHED THE FILL AREA & THEN CAUGHT ON DRY MATERIAL CAUSING TRUCK TO FLIP. EE WAS NOT WEARING SEAT BELT AND HE WAS THROWN FROM TRUCK INFLICTING BRUISES ON HIS HEAD, NECK, CHEST AND LEFT LEG.
