EMPLOYEE STATES THAT HE WAS REPLACING RADIATOR ON EXCAVATOR. THE TRIGGER THAT LOWERS THE WINCH MALFUNCTIONED CAUSING IT TO LOWER TO QUICKLY. HE TRIED TO GRAB THE RADIATOR AND HURT HIS LOWER BA CK. SUPERVISION WAS NOTIFIED ON 10/20/99.

AFTER HE LOADED THE TRUCK IT STALLED. BY THE TIME HE HAD THE TRUCK RE-STARTED, HE WAS ALREADY IN THE CURVE WHERE THE ACCIDENT TOOK PLACE. HE DID NOT HAVE ENOUGH TIME TO REGAIN CONTROL OF THE TRUCK.
