EE WAS USING A HYDRAULIC JACK TO RAISE THE BED OFF THE FRAME ON A MECHANICS TRUCK. THE JACK KICKED OUT & THE BED DROPPED ONTO THE FRAME (APPROX 2-4" DROP). THE BED HIT HIS NECK/SHOULDERS.

EMPLOYEE WAS OPERATIN GROCK TRUCK WHILE IN PIT HE WAS TOO CLOSE TO HIGHWALL. A LARGE ROCK FELL FROM THE HIGHWALL & STRUCK THE CAB PROTECTION ON THE RIGHT SIDE. THIS CAUSED THE OPERATION TO B E JERKED RATHER HARD. HE WAS REMOVED & TRANSPORTED COMPLAINING OF LOWER BACK PAIN.

EE WAS SWEEPING THE TRUCK CAB WHILE WAITING TO BE LOADED. HE HEARD A ROCK FALL FROM THE HIGHWALL & IT STARTLED HIM. HE JUMPED, BANGING HIS HEAD ON THE DOOR. THE DR. TOLD HIM TO TAKE A DAY OFF BECAUSE HE WAS HAVING DIZZY SPELLS.

EE WAS CHECKING AN OIL LEAK. THE DOOR SLIPPED OFF ITS HINGE CAUISNG HIM TO LOSE HIS BALANCE AND FALL TO THE GROUND. HE BRUISED HIS NIGHT HIP AND ELBOW.
