Showing all 27Was riding on man trip with jacket sleeve hanging outside of ride. A tight corner was turned and the jacket became caught between the ride and rib pulling his right arm between the ride and the rib causing a fracture to his right forearm.
Employee was unpinning sub and rectifier from a tractor, the pin was in a bind so he asked the operator of the tractor to move backward. When the tractor rolled backward the pin caught the employee's right hand causing a laceration to his hand. This resulted in six stitches to repair the hand.
EE was on a new set up pulling cables, a hauler pulled a shuttle car into a xc to be spotted. He went to unpin the tongue from the car, but the pin was bent so he went to the hitch on the hauler to try and unpin it there when he tried to pull the pin out the car rolled causing the pin to move mashing his Lt ring finger between the pin and the counter weight of the hauler.
Employee was sitting on a mantrip with his right leg outside of the mantrip. The water car was being backed into entry and Employee was not seen, resulting with right foot being run over by water car.
Employee was walking belts when he attempted to remove some belt fibers that had wrapped around edge of roller. He pulled on fiber & when it got tight the roller grabbed fiber & pulled his thumb into roller resulting in injury.
EE was putting tools on Tool Slide, when a Golf Cart pulled in xc & struck EE on right leg, just below knee. The drivers dinner bucket had slid behind the brake pedal, & driver couldn't stop Golf Cart resulting in accident.
Employees feet was resting on the outer bottom frame of the ride. When the mantrip made a left turn, his right foot was caught between the rib and the frame. Resulted in a fracture of the big toe and portions of his other toes avulsion. He had surgery which resulted in his big toe, second toe, and third toe being partially amputated and grafted.
Employee was riding in a covered mantrip facing forward in the seats located behind the right side. The mantrip ran over a roof bolt, causing it to strike him below the left eye. This resulted in a laceration below his left eye.
Injured employee was setting in the rear seat operators side of an A.L. Lee 10-Man ride. When the employee decide to put on his long sleeve shirt, while the ride was moving going under an overcast striking his left hand resulting in a fracture.
Employee was driving a stinger ride when the front tire hit a rock, causing the steering wheel to turn and hit his right hand. This caused a fracture to his right hand.

Employee was placing pinboards in the scoop bucket when a shuttle car hit the rear end of the scoop. The scoop moved to the side of the employee causing the scoop bucket to hit the employee's foot.
Employee was pulling the left bolter cable. He had the cat head in the ride he was using when the cable hung on something catching his hand between the cat head and the ride.
Employee was assissting the section mechanic splice his car cable, when a scoop came through a ventilation curtain and struck the employee, causing him to be shoved underneath the shuttle car. The result of this accident was fractured ribs and a bruised liver.
Employee was in the process of pulling cable from the reel of his shuttle car to allow a scoop to hook to the car and pull it from a mud hole. The cable was struck by the scoop bucket causing it to arc which resulted in flash burns to the employee's eyes and he was given prescription medication as a result.
Employee was riding on a Wallace diesel mantrip, The mantrip was traveling on the 1st North Supply road.The mantrip was going through the most inby air door when the air door inadvertently closed and mashed the employees foot against the mantrip causing 3 toes to be fractured and resulting in the little toe on his left foot to later be surgically removed.
Employee was talking to another employee who was operating a mini trac. A third employee was loading crib blocks on a golf cart when he accidently placed one on the tram pedal of the golf cart. This caused the golf cart to start tramming and hit employee on the legs pinning them against the mini trac.
EMPLOYEE WAS OPERATING A 10-MAN DIESEL RIDE, AS HE WAS BACKING UP HE RAN OVER A ROCK THAT CAUSED HIM TO HIT HIS HEAD AGAINST THE MINE ROOF.
EMPLOYEE WAS DRIVING A SHUTTLE CAR BEHIND A MINER THAT WAS CUTTING UP BOTTOM, AS THE EMPLOYEE WAS TURNING THRU AN INTERSECTION, HE SAID A PIECE OF ROCK SLID IN HIS DECK AND HIT HIS HAND OR FELL FROM THE ROOF AND HIT HIS HAND, HE WASN'T SURE.
THE #1 UNIT MANTRIP WAS RETURNING TO THE OUTSIDE AFTER THE SHIFT WAS OVER ON THE N. WEST HAUL ROAD WHEN THE MANTRIP HIT A SET OF ARCHES. WHEN THE MANTRIP STOPPED SUDDEN EE HIT HIS HEAD ON THE MANTRIP. HE WAS NOT WEARING HIS HARD HAT.
EE WAS REMOVING A STEEL BEAM IN A PREPARATION PLANT. THE PIECE OF STEEL WAS BEING SECURED BY A TRUCK MOUNTED CRANE. AS THE LAST BOLT WAS CUT LOOSE THE STEEL SWUNG & HIT HIM ABOVE THE LEFT EYE.
