The employee finished his shift (owl) and was on the haul road going home. The coal trucks were on the same road going to the mines. The two vehicles collided and mine employee's car was hit on the driver's side by the bumper of the coal truck.

Showing all 11Employee was in scoop bucket getting ready to ride to face area. He had hand on top of ram scoop. Operator got in scoop and activated ram lever not knowing injured had hand in pinch point location.
The employee was positioned at the rear of the section feeder observing a stalled feeder chain. A parked coal hauler rolled down a grade, catching the EE between feeder & hauler resulting in injuries requiring amputation of right foot & ankle & fracturing the left leg. The accident is still under investigation.
Was riding rail runner to section. Got cramp in leg. Raised up, too high. Hit mine roof. Knocked him back down in rail runner, he hit frame of rail runner with left hip.
Miner foreman crawled out into intersection to look, swag was in area, ram car came over swag. Foremen did not see car. The car artickiled down, bucket end hit foreman's right leg.

ROAD WAS WASH BOARDED & DRIVER DEPLETED AIR OFF BRAKE SYSTEM. THE TRUCK BEGAN TO RUN AWAY & THE DRIVER JUMPED FROM THE TRUCK, STRIKING THE GROUND.
FOREIGN MATERIAL WAS BLOWN BEHIND EE'S SAFETY GLASSES INTO HIS RT. EYE. AS HE STARTED A BELT DRIVE. HE DID NOT MISS ANY WORK DUE TO THE INJURY, BUT PRESCRIPTION MEDICATION WAS USED. NO MATE RIAL WAS IN HIS EYE WHEN HE SAW A DR.
EE WAS OPERATING A SHUTTLE CAR. AS HE WAS TRAVELING UP THE ENTRY, HE PLACED HIS HAND ON TOP OF THE CAR CANOPY AND HIS FINGER WAS CAUGHT BETWEEN THE CANOPY AND THE MINE ROOF. FRACTURE AND LACER ATION TO LEFT RING FINGER. RETURNED TO REGULAR DUTIES NEXT SCHEDULED SHIFT.
