Employee was exiting the mechanical sampler building to change out his sample bag. As he was returning to the prep office he slipped/fell on left knee. The slip was due to sludge falling from the washdown of the rail unloading hopper. He worked every shift with no restrictions until his condition became painful and his doctor schedual surgery 01/15/07. Pre-exisiting knee injury.
THE EE WAS ATTEMPTING TO DISLODGE THE STEEL CABLE AND "D" RING FROM A SHEEVE WHEEL. THE CABLE CAME OUT OF THE SHEEVE KNOCKING HIM OVER THE RAILING INTO THE OPEN HOPPER BARGE APPROX 12 FEET. HE LANDED IN 12" OF WATER ON A STEEL BOTTOM OF THE BARGE.
WHILE CLIMBING DOWN FROM THE OPERATOR COMPARTMENT OF THE LOADER, THE EE SLIPPED FROM THE SECOND STEP (APPROX 35" FROM THE GROUND) HE SLID UNDER THE LOADER STRIKING HIS RIGHT SHOULDER.
