He was repairing bridges and installing stone in tramway, he twisted his left knee, causing a left MCL strain and derangemant.
The injured tripped over the anchor chains for the belt tailpiece and fell against a belt stand and injured his right shoulder. The injured continued to work till 8/6/09 when he went to the doctor and was diagnosed with a torn rotator cuff. His 1st day off work was 8/7/09
Employee was exiting a truck when he slipped on the running board causing a meniscus tear to the right. ***This is a reoccurance***
Washing down sump with a water hose while standing on a platform. Slipped and fell approx 18" striking back on a crib block and handrail.

While cleaning the 3A belt head employee tripped over a turnbuckle and fell on a steel beam on his right hand (13 stitches to his index finger)
The injured slipped while walking up steps in the Prep plant. When he caught himself he felt pain in the right shoulder. The injured has had surgery to this same shoulder in 1991. The injured continued to work till 8/26/09 when he had surgery to repair degenerative damage to his right shoulder
While carrying a belt roller to the belt line, employee stumbled on rock and fell down. Roller came down on his right hand. (6 stitches to middle finger and 4 stitches and fracture to the ring finger.
Removing belt structure while standing on a ladder. He fell off and landed on his right side. He continued to work until 5/1/09 at which time it was discovered he had fractured several ribs.
Injured was walking around a diesel mantrip doing a pre-operational inspection and slipped on a piece of coal. He fell and landed on his right hand fracturing it.

He was walking along the stage loader slipped in the mud and twisted his left knee. He continued to work at his regular duties but continued to have pain. He was removed from work for surgery to repair a torn miniscus of the left knee on 6/11/09.
Injured was stepping off a ladder and stepped on a piece of concrete block, straining his right ankle. He began missing time on 02/16/2009.
Victim was walking between scoop and rib and tripped on scoop bucket, falling to the mine floor. He broke one tooth off an dloosened two other teeth in the front of his mouth. This is a medical treatment injury.

EE was in the process of blocking supply cars in order to place them back on track after they had derailed. He stood up quickly, causing him to strike his head on the steel beam used to support a ventilation door on the track. This caused him to loose his balance and fall, face first, into his 6 ton motor. Sutures required to close laceration above his right eye.
EE was climbing a set of steps when he fell causing a rupture to the quarraceps tendon of the right leg. He was scheduled for surgery on 9/17/08.
Employee slipped on scaffolding pick, grabbing beam with right hand to regain balance injuring his right shoulder. He was tied off.
EE originally reported as not sure where the knee injury occurred & turned it in on insurance. He later was asked by the knee specialist if it could have happened at work & ee said it could have. He remembered some pain in knee as he exited his Uni-Hauler once. Since no one can determine the event, we wish to revise this report to remove the LTA.
At the time of the incident the tech was installing an oil filter base on a CAT 793 truck. As the tech was starting the first bolt the ladder he was standing on partially folded up causing him to quickly step off the ladder. On the way off the ladder the tech's left hand was caught between the fill tube and the filter housing and lacerated.
EE felt pain in his right knee when exiting a motor. This occurred on 9/2/08. He continued to work at his regular duties, but continued to have pain. He eventually was removed from work for surgery to repair a meniscus tear of the right knee on 10/23/08.
He was walking to his work site, stepped in a hole and felt pain in left knee. He continued to work at his regular duties but continued to have pain. He went to a physician and was removed for surgery on 11/20/2008 to repair a tear of the medial meniscus.
The injured said he twisted his knee slightly while walking. He then twisted it again while stepping off an offset, causing him to fall backwards. Upon a doctor's exam it revealed degeneration of the medial and lateral compartments of the left knee. The injured suffered pain from an occupational illness. The injured's first missed day was 8/13/2008.
He was carrying a compressed gas cylinder and caught his left foot on a rail, twisting left knee. Injured continued to work for 16 weeks. A physician diagnosed degeneration in his left knee. He began losing time from work for surgery on 11/26/2008.
While carrying belt structure to track entry, EE got his foot stuck in mud and twisted his back.

He eventually went to a physician and was diagnosed with severe degenerative joint disease. This is occupational illness due to repeated trauma. He was removed from work on 9/12/2008.
Employee was walking and tripped over hole in mine. He slipped and sprained his knee.
During set up for repairs to a 4100 shovel, employee stepped off of the 24" high cribbing ramp onto scorio pad. He twisted his left knee as he stepped onto the loose scorio.
