Employee stepped into a 14" wide by 3' long hole with a 6" drop to the new beam which had to be open due to construction. Employee caught himself on a 3' column that had a sharp corner causing a puncture wound to his left forearm. The wound required 3 stitches inside and 9 stitches outside.

Employee was dismounting a dozer by stepping onto the track, and onto the push arm. As he was stepping from the track to the push arm, his left foot slipped and he tried to catch his fall with his left hand. The hand struck a piece of raised metal installed on the push arm for foot traction.

Injured employee had stopped by the Warehouse prior to the start of his work shift to pick up some ear plugs. As he was walking between his personal vehicle and the Warehouse, he stepped on some ice and fell on the parking lot surface. The fall resulted in a fracture of his left femur bone.
