Miner was cleaning/shoveling excess feed from the around the mobile rinser, loader operator placed material into hopper when a compacted segment slid from grizzly grid striking miner. Miner lost EE's balance / footing falling to the ground. During chain of events after being struck and coming to rest on the ground, miner sustained a fractured femur on EE's left leg.
Employee was walking to raw feed area to collect a sample for quality control. While walking, employee tripped in mud and fell down. First aid was performed. Employee went to an occupational clinic on July 16, 2018, where employee was treated for a sprained right knee.
Plant operator stepped onto grating. The grating shifted, causing the operator to trip and fall. First aid was provided. Employee went to an occupational clinic on July 18, 2018, where ee was treated for a sprained right knee.
Crane mechanic was working on the Link Belt Crane pulling the air cleaner. EE stepped back thinking EE was stepping onto the main deck from the catwalk. EE miss-stepped, missing the main deck and fell to the ground level. EE was not wearing fall protection.

Technician was on on-call weekend and was unloading and placing barrels. The technician had to cross over a concrete transformer pad. There was a hole that was not covered and the area was poorly lit. As the technician turned around the technician missed footing and tripping on the edge of the hole and foot going into hole.
Employee was working on removing ice as he stepped off the last step on the ground level set of stairs with my left foot and slipped on a flat sheet of ice. Investigation Findings: At the bottom of the stairs on the line 1 diester there was a layer of packed snow and ice covered by water as the ice was melting. There were no signs of ice or snow on the stairs or hand rails.
Employee was getting air filters off the warehouse rack. He was using the 5 foot mobile stairs instead of the 8 foot stairs. when he reached for the filters on the top shelf at a height of 12 feet he lost his balance and fell backwards off the stairs, landed on his right heel. Employee has a broken heel
Employee was walking to the stacker belt after a heavy rain storm, there was an accumulation of water he tripped fell forward and hit his head on the metal frame, leaving a red mark, there was no excessive swelling, no loss of conscience.
After getting out of loader, operator turned and twisted right knee.
While clearing walkways of snow and ice; slipped and fell.
Employee was stepping down off rail car with left leg and spraining left ankle.
Employee was working in a dozer when he decided to exit the vehicle. At this time he caught his left foot on the foot rest and twisted his left knee. Employee hurt his knee on the 5th and sought medical attention on the 6th of November.
