Employee was washing rocks off of the truck scales at the bins. The employee was stepping down from the concrete around scales to a flat surface and felt a "pop" in right knee. Employee was diagnosed with a right knee sprain and placed on restricted duty.

After exiting man lift, employee stepped on uneven ground and twisted his right knee. Employee had it checked out medically on 11/17/2015.

Injured employee sprained his left ankle as he jumped down from the CAT 775D haul truck to extinguish a fire. After the original treatment of the injury, the employee required an additional medical procedure to treat swelling/blistering that had developed in the area that was splinted. The fire was extinguished in less than 10 minutes.

Employee was entering the cab of a 775B cat rock truck and stopped on the step and fell to the guard. Employee landed on his feet but the impact broke his left ankle

After clocking out at the end of his shift, employee decided to stop at the bulk diesel tank to pour the remaining 1-2 gallons of fuel treatment from a 25 gallon keg into the bulk diesel tank. While on top of the tank pouring the material in, the employee slipped and fell to the ground striking his head. This resulted in a laceration requiring 9 staples and a concussion.

Employee strained his lower back / pulled a muscle when stepped off of a crusher platform. The platform is 12 - 14 inches higher than the surrounding ground level.

Crane operator was inspecting the winch (drum) on the crane. After inspecting the winch, the operator was climbing back to the cab of the crane when he slipped and fell approx. 8' to the ground below. Operator sustained laceration to his head and a fracture to his hip and wrist.

Employee was trying to remove a rock that was jammed between the conveyor frame, the belt and the return roller. Employee was using a pry bar and while he was trying to remove the rock the bar slipped. The employee lost his balance and while he was trying to catch himself he strained his left knee.

Employee was making repairs to an excavator. He reported that he began to feel ill, became nauseated, then "blacked out" and bumped his head on the equipment causing a laceration which required stitches. It is believed heat exhaustion may be contributing factor and corrective actions are being put in place to prevent recurrence.
