The drill operator was assisting on drill maintenance, EE was guiding the drill hammer which was suspended by the mechanic truck boom when EE tripped over a rock, that had not been removed from the work area. The operator stumbled while maintaining EEs grip on the drill hammer, between EEs grip on the hammer and EEs shifting body weight resulted in a fracture to the left scaphoid.
While descending the steps from the top level of the plant, the Miner slipped with wet boots causing EE to hit head below EE's hardhat strap. When EE reported the incident to the Foreman, EE became ill and vomited. The supervisor called emergency services.
A contractor entered the site during non-working hours to retrieve a personal car spring from EE's personal storage trailer. A guest that was accompanying the contractor heard a thud as back was turned towards the trailer. The guest turned and found the contractor lying on the ground with head injuries.
After filling the radiator with fluid on the Epiroc D65 Drill and returning the tools to the grease truck, the Miner returned to replace the radiator cap. Mud on Miner's boots caused the Miner's feet to slip and fall from the ladder landing on right leg, spraining knee.
