INJURED STATED THAT HE WAS SITTING IN THE MANTRIP IN THE REAR COMPARTMENT, FACING IN THE OPPOSITE DIRECTION OF TRAVEL, WHEN THE NEXT THING HE KNEW, HE WAS LAYING THE TRACK AND THE MANTRIP DROV E AWAY. THE MANTRIP (RAILRUNNER) WAS TRAVELING TO THE SURFACE WITH A CREW ENDING THEIR SHIFT. NO ONE KNEW WHEN INJURED LEFT THE MANTRIP AS OTHERS WERE SITTING NEXT TO HIM. INJURED SUFFERED A
INJURED STATED THAT HE WAS WATCHING THE CONTINUOUS MINER CABLE AND WATERLINE AS THE CABLE WAS BEING PULLED DOWN THE BELTLINE. HE SAID HE TOOK HIS EYS OFF THE CABLE BRIEFLY TO GET A BOLTING MAC HINE WRENCH THAT WAS LYING BY THE BELTLINE. THIS IS WHEN A LOOP OF CABLE CAME BY AND DRAGGED HIS FEET FROM UNDERWRITH HIM, CAUSING HIM TO FALL.
INJURED STATED HE SLIPPED & FELL ON THE SLICK BOTTOM AND TWISTED HIS ANKLE.
INJURED STATED THE HE WAS PULLING THE CONTINUOUS MINERS CABLE WHEN HE SLIPPED IN THE MUD AND TWISTED HIS KNEE. INJURED HAD NO LOST TIME UNTIL 2-7-03. AT THIS DATE HE TOOK OFF FOR MINER KNEE SU RGERY. HE IS SCHEDULED TO BE OFF WORK 4-6 WEEKS.
THE INJURED WAS STEPPING OFF THE STEP OF THE SERVICE TRUCK AND STIPPED AND FELL AND STRAINED HIS SHOULDER.
THE INJURED WAS WALKING OUT OF THE FILTER TRAILOR DOWN THE RAMP AND STEPPED ON A ROCK CAUSING HIM TO TRIP AND FALL.

INJURED HAD TRAMMED THE BOLTING MACHINE TO THE 1LEFT CUT TO BEGIN BOLTING. HE WAS IN THE PROCESSOF SETTING THE ATRS WHEN THE TOP BOOMED & FRIGHTENED HIM. AS HE TURNED REAL FAST TO RUN, HE FELL ONTO A ROCK AND CHIPPED A BONE IN HIS LEG JUST BELOW THE KNEE.
THE INJURED WAS CROSSING THE BELT AT THE TAILPIECE. AS HE JUMPED FROM ONE SIDE TO THE OTHER, AS HE LANDED, HE TURNED HIS ANKLE.
THE INJJURED STATED HE WAS FOLLOWING THE ROOF BOLTER ACROSS THE SECTION & SLIPPED IN THE MUD AND FELT A PAIN IN HIS LOWER BACK.
THE INJURED WAS CLIMBING OFF THE FRONT LOADER WHEN HE MISSED THE 3RD STEP AND FELL APPROX 5' TO THE GROUND LANDING ON HIS RT KNEE.
THE INJURED WAS CLIMBING OFF AN OVERCAST USING CONCRETE BLOCKS TO STEP OFF ON WHEN THE BLOCKS TURNED OVER CAUSING HIM TO FALL AND TWIST HIS ANKLE.
INJURED HAD PLACED THE BOLT HALFWAY INTO THE HOLE HE HAD JUST DRILLED AND AS HE ATTEMPTED TO MANUALLY STRAIGHTEN THE BOLT IN ORDER TO SHOVE IT THE REST OF THE WAY INTO THE HOLE, HE SLIPPED AND FELT SOMETHING PULL IN HIS LOWER BACK.
THE INJURED WAS STEPPING OF THE DRILL TO CHECK BATTERY WHEN HE SLIPPED ON SOME OIL AND FELT A PAIN IN HIS LOWER BACK.
INJURED STATED THAT ON 6-2-00 HE WAS HANGING A BLOCKCURTAIN AND TWISTED HIS KNEE. HE HAD NO LOST TIME UNTIL 8-10-00 AT WHICH TIME HE WENT TO THE DOCTOR.
