Employee stated that EE was clearing out trees/brush for power line. EE's left foot slipped on the bank causing EE to fall onto the ground. EE hit the right side of EE's face against a stob causing 2 small lacerations above and below EE's right eye. This is a no lost time accident but reportable due to requiring sutures.

Showing all 49Employee stated that EE was getting off of dozer. The push beam was up on jacks. As EE stepped down off of push beam step, EE felt EE's right knee buckle when EE stepped onto the ground causing pain in EE's right knee aggravating a previous injury.
Employee stated that ee was dismounting dozer at the end of shift. EE stepped on a rock which was laying on the step of the push beam. EE foot slipped off causing ee to fall to the ground and landing on right foot. EE said ee felt knee pop which caused severe pain. Went to doctor next day and is off work.
Employee stated was climbing up ladder to get on hauler. EE was standing on the bumper or step above so as to set thermos onto the catwalk. As EE did, hand slipped off the hand rail and EE fell backwards onto the ground. EE landed on back resulting in nine fractured ribs to the right side.
Employee was walking across shot rock to the foreman's truck when their right foot slipped off of a rock and into a crevice. This caused the foot to bend backwards. Went to ER that night to get checked due to increasing pain. X-rays were negative but lost time started on 07/25/17.
Employee was pulling hose from fuel truck to fuel dozer. Foot got stuck in the mud causing employee to lose balance. Employee fell backwards into the rear bumper on the fuel truck. Employee hit back against the bumper causing back pain and sprained left wrist when employee tried to break the fall.
Employee stated that was servicing a dozer. EE reached to loosen fuel filters and twisted left knee. Immediate pain but lessened with time. Kept working. Went to doctor on 5/23/16 to get checked. Never missed time/work until 7/15/16 when EE had orthoscopic surgery on knee and incurred lost time.
Employee stated that he was patching the supply pipe to the distribution box on the #70 screen. The ladder slipped causing the employee to lose his balance and fall onto the vibratory screen. This resulted in a right sprained ankle and bruising to the left leg/groin area.
Employee stated that he was at the shop to wash his truck. He grabbed the hose and was backing up when he stumbled/fell over a drive line laying on the floor. Fell backwards landing on right shoulder. Did not go to doctor until 9/26/14 due to recurring pain. Tendon pulled in right shoulder. Lost time starting on 10/17/14 due to surgery on shoulder, thus reportable.
Employee stated that he was removing the bottom pin from the steering cylinder on the loader. As he was coming down off of the step ladder onto the ground, he stepped onto uneven ground causing him to sprain his right ankle. Missed two shifts of work.
Employee stated that he was operating dozer in the shot when he got off for a moment. When he was getting back on the dozer, his foot was on the step on the push beam and his hand was on the hand hold. As he was pulling himself upon the track, his hand slipped off causing him to fall back onto the ground. Had some back pain, went to ER, and missed one shift of work on 5/2/2014.
Employee had been welding on the rear cross member of main frame on a hauler. He had finished welding and said he was in the process of getting down the ladder when his arm let go and he fell onto the ground striking his head against the ground incurring a concussion and also had shoulder pain.
Employee stated that he was pushing trees down with dozer. A tree dented engine compartment door. He was going to lay door off of dozer onto ground. Standing on tracks, pried on door, door came loose, employee lost balance and jumped onto ground. Hit left hand on push beam fracturing left ring finger.
Employee stated that he was reaching up to open the crusher on the sample system at the strip dump. As he was pulling down on the lever to open it up, he turned his left ankle.
Employee stated that he was getting out of his mechanic's truck. As he stepped onto the ground, he stepped onto a ridge of dirt and sprained his right ankle.
Employee stated that he had started up a step ladder to work on a front end loader but stepped back off to get a tool. As he stepped off, his right foot stepped onto uneven ground causing him to sprain his right ankle. No lost time incurred but reportable due to restricted work duties.

Employee stated that he was getting out of his mechanic's truck. As he put his feet on the ground, they slid out from under him causing him to fall and strike his head against the floor of the truck. This resulted in a cervical strain and headache.
Employee stated that he was preparing to put a tooth on the loader bucket. He stepped on some ice on the ground, slipped, fell and caught himself against the bumper on his mechanic's truck. He twisted his back resulting in low back pain.
Employee stated that he was getting onto a dozer. He had one foot on the step of the push beam and the other foot on top of the push beam. His right foot slipped off of the step causing him to fall down and strike his right rib cage against the top of the push beam resulting in a fractured right rib.
Employee stated that he was coming down the steps on the loader when his foot slipped off the step. He grabbed hold of the hand hold on the side of the engine compartment to prevent him from falling. As a result, he pulled a muscle in his left shoulder.
Reported 9/8/10 - attempting to hook up hose from tanker to tractor to blow off product, slipped and fell backward hit catwalk and landed on tail bone.
Employee stated that he was working on a D10 dozer. As he reached to get tools, his foot slipped/stumbled on some small rocks laying on ground causing him to twist his right knee. Not reportable until he lost work on 9/15/10 due to doctor visit and starting 9/24/10 due to orthoscopic surgery on his right knee.
Employee stated that he had just checked the oil levels in the motor grader and was starting to get into the cab when his foot slipped off of the tire and he fell approximately 5 feet onto the ground injuring his right shoulder.

Remounted loader and proceeded to work. Felt pain in left leg later. Went to doctor, checked out and returned to work. Became reportable when he missed work on 8-24-09.
Employee stated that he was getting out of the dozer. He stepped on a loose bolt on the deck causing him to slip. He grabbed hold of a safety rail to keep from falling. As he did, he strained his shoulder. Taken off work on 4-9-09 to have minor shoulder surgery to repair rotator cuff tear.
Employee stated that he was starting down the ladder on the loader. When he grabbed for the right hand rail, he missed it. His left hand was on the left rail and caught his body as he slid down the ladder onto the ground. He felt a pop in his left shoulder.
Employee stated that he was training for the position with the lube crew. While helping service a loader, he sat down on top of the engine hood attempting to take his coveralls off. He said he fell backwards off the loader and down between the rear tire and loader striking the axle.
Employee stated he was getting onto dozer when left foot slipped off step. He had hold of grab iron which prevented fall but it strained right shoulder. Hurting but seemed ok. Went to doctor later. Found small tear in shoulder. Opted to keep working. Finally decided on minor surgery to repair tear. Did so and lost time starting on 11/17/08, thus reportable.
Employee stated that he was disconnecting fuel hose from fuel truck. As he did, diesel fuel sprayed out on him. He stepped back to get out of the way. As he did, he tripped on some rocks, fell onto ground striking left elbow on ground causing laceration.
Employee stated that he was standing on his dozer cutting a fallen tree off of another dozer. When the tree was cut, it pushed him off of the dozer causing him to fall onto the ground resulting in some abrasions, contusions, and some back pain.
EE stated that he was coming down the steps from 2nd to 1st floor in the prep plant. EE's right foot slipped, causing him to fall against the handrail. As he did, he twisted his left knee.
Stepping out of the truck, felt pain go down left side of back & down left leg.
Employee stated that he was walking down the steps on a loader. His right foot slipped on next to last step causing him to lose his balance. He was holding onto the handrail and as he started to fall, he gripped the rail tighter causing him to sprain his right wrist.
EMPLOYEE STATED THAT HE WAS COMING DOWN THE STEPS AT THE PARTS TRAILER. THE BOOT ON HIS RIGHT FOOT GOT CAUGHT ON THE SECOND STEP, CAUSING HIM TO FALL FORWARD TOWARD THE GROUND. HE LANDED ON HI S LEFT FOOT CAUSING IT TO CREEL UNDER RESULTING IN FRACTURE ABOVE THE ANKLE ON HIS LEFT FOOT. REPORTABLE BUT NO LOST TIME INCURRED.
EMPLOYEE STATES HE WAS LOADING A SHOT AND LOOSE MATERIAL BEGAN TO FELL OF WELL. WHILE TRYING TO ESCAPE, HE FELL ON ROCKS CAUSING A CONTUSION TO HIS RIGHT ARM AND RIB CAGE.
EE STATED THAT HE WAS WORKING ON A HYDRAULIC PROBLEM WITH THE DRILL. HE STEPPED ONTO THE COVER OVER THE DRIVE LINE AND DOWN ONTO THE OTHER SIDE. AS HE DID, HE STEPPED ONTO SOME HYDRAULIC HOSES ON THE DRILL CAUSING HIM TO TWIST HIS LEFT ANKLE. NO LOST TIME INCURRED BUT IS ON RESTRICTED WORK ACTIVITY.
EE STATES: CLIMBING UP LADDER ON TANKER WHEN FOOT SLIPPED OFF REAR BUMPER CAUSING TOES TO BEND AND HEAD CAME FORWARD CAUSING NECK PAIN.
EE STATED THAT HE WAS STANDING ON THE BUMPER OF THE MECHANICS TRUCK WITH ONE FOOT ON THE PUSH BEAM ON THE DOZER. AS HE WAS HEANDING AN OIL CAN TO THE MECHANIC ON THE DOZER, HIS FOOT SLIPPED CA USING HIM TO FALL AND HIT HIS LEFT HIP AREA TAILBONE ON THE PUSH BEAM. DID NOT GO TO DR UNTIL 3-21-02 AND WAS ONLY REPORTABLE WHEN FIRST LOST WORK DAY OCCURRED 4-5-02.
EE STATED THAT HE WAS CARRYING A BEARING CARRIER IN HIS RIGHT HAND AND SEALS IN HIS LEFT HAND WHILE WALKING DOWN SOME STAIRS IN THE WAREHOUSE. TOWARD THE BOTTOM OF THE STAIRS, HIS FOOT SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND FALL. HE FELL ONTO THE STEPS, LANDING ON HIS RIGHT ARM AND POSTERIOR.
EE WAS DESCENDING THE LADDER ON THE BACK OF THE OIL TRUCK AFTER HE HAD FILLED IT WITH OIL. HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE LADDER & HE CREELED HIS ANKLE ON THE GROUND.
EE STATED THAT HE WAS STEPPING DOWN ONTO THE GROUND FROM THE PUSH BEAM ON THE DOZER. AS HE STEPPED ONTO THE GROUND, HE STEPPED IN A HOLE AND TWISTED HIS RIGHT ANKLE.
EE STATED THAT HE WAS CHANGING OUT THE TOP SCREEN ON THE VIBRATOR DECK. HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL/SLIDE DOWN OFF OF THE SCREENS. HE LANDED ON HIS LEFT KNEE. NO LO ST TIME WAS INCURRED BUT IS REPORTABLE DUE TO BEING ONSOME RESTRICTED WORK.
EE STATED THAT HE WAS CLIMBING UP THE HAULER STEPS TO CHECK THE OIL LEVEL AND TO START UP THE HAULER TO WARM IT UP. AS HE WAS CLIMBING THE STEPSHE MISSED THE HAND RAIL AND FELL BACKEWARDS AND ONTO THE GROUND(APPROX FALL OF 7 TO 8 FT.) CAUSING A COMPRESSION FRACTURE TO THE T12 VERTEBRAE.
EE STATED THAT HE WAS GETTING ON THE DOZER TO WORK. HE STEPPED UP ONTO PUSH BEAM STEP WHERE COAL WAS PILED ON TOP OF IT. HIS FOOT SLIPPED OFF CAUSING HIM TO HIT HIS LEFT KNEE AGAINST THE STEP. KNEE STARTED SWELL8ING ABOUT 3 WEEKS LATER. WENT TO DR. ON 1-3-2001. NOT REPORTABLE UNTIL LOST TIME INCURRED ON 1-31-2001.
EE STATED HE WAS OPERATING A DOZER ON 2ND SHIFT HE STOPPED THE DOZER TO CHECK AN OUTSIDE LITE WHICH HAD STOPPED WORKING AS HE WAS STANDING ON THE DOZER TRACK TO CHECK THE LIGHT, HE FELL OFF OF THE DOZER IN BETWEEN THE PUSH BEAM & ROLLER FRAME OF THE DOZER. MISSED 1ST WORK SHIFT ON 10/19/00
EE STATED THAT HE STEPPED ON A ROUND BOLT LAYING ON THE FLOOR OF THE SHOP BUILDING. HIS LEFT FOOT SLIPPED ON THE BOLT AS IT ROLLED CAUSING HIM TO FALL. HE CAUGHT HIMSELF WITH HIS HANDS. HOWEVE R, AS HE DID, THIS CAUSED HIM TO TWIST HIS BACK RESULTING IN LOWER BACK PAIN, DID NOT GO TO DR. UNTIL 9-20-00 WHICH RESULTED IN LOST WORK DAY AND RESTRICTED WORK.
EMPLOYEE STATED THAT HE HAD FINISHED HIS WORK SHIFT AND WAS EXITING HIS HAULER. HE STATED THAT HE HAD NOT HIT OR TWISTED HIS KNEE IN THE PROCESS. HOWEVER, AS HE WAS CLIMBING DOWN THE HAULER LA DDER HE FELT A BURNING SENSATION IN HIS RIGHT KNEE. AFTER REACHING THE GROUND, HE STATED THAT THE BURNING EASED OFF. ON 8-23 HE MISSED WORK DUE TO KNEE AND WENT TO DOCTOR.
EE STATED THAT HE WAS DISMOUNTING THE DOZER. HE STEPPED FROM THE DOZER ONTO THE PUSH ARM AND THEN FROM THE PUSH ARM ONTO THE GROUND. AS HE STEPPED ONTO THE GROUND, HIS LEFT FOOT STEPPED ONTO F ROZEN MUD WHICH CAUSED HIM TO TWIST HIS ANKLE AS HE WAS STEPPING DOWN.
