An operator was filling a tank on truck #40. The operator's foot slipped off the edge of the platform causing them to slip, and they fell into a handrail, breaking three ribs on their left side. The other operator they were with immediately came over to the scene, laid them down and called for help.

Mechanic stepped off the service truck without using three points of contact. Mechanic descended the service truck steps going forward instead of in reverse which is the requirement with diesel fuel nozzle in one hand. When EE's left foot stepped off the ladder and contacted the ground, EE rolled and twisted EE's ankle causing a fracture to left ankle.

Employee slipped on the wheel chock berm at ground level while servicing a FEL. The employee landed on EE's lower right back area and experienced a bruise/contusion to the area.

Employee was using a pry bar to remove a loader tire from the rim the bar slipped. Employee stepped backwards, and when EE did EE stepped onto the fork of a fork truck behind EE. The fork had mud on its surface. When EE's foot stepped on to the edge of the fork EE's ankle rolled and EE fell. EE's ankle was fractured as a result

While employee was clearing snow from a small pile of fines that had been left by a loader operator by the grinding room earlier for sampling, the employee moved to the left while doing so left foot slipped out from underneath employee on some snow covered ice that employee did not realize was there. Employee twisted knee and fell into the fines pile.

At approximately 3:40pm, EE was dismounting haul truck #718's flexible ladder and missed the last step to the ground. EE was not using 3-point contact at the moment left foot missed the step. EE reacted quickly, and grabbed the handrail with right hand to prevent ee from falling backwards

Employee was torquing track roller bolts on a D8-R dozer when torque wrench internal gears stripped out of ratchet head of wrench. Employee was in squatting position when torque ratchet failed causing employee to lose balance and strike the floor hitting elbow.

While conducting a pre-shift inspection, the employee climbed off of a front end loader and turned to go check the next piece of equipment and rolled his left ankle causing him to fall. The employee just received first aid treatment. On 9/30/14 his ankle was still bothering him so he was taken to the doctor where he was prescribed a steroid med and released back to full duties.

Employee had finished loading ANFO and was in the process of wrapping up the on/off control valve hose. Employee's feet got caught in the hose and he fell to the ground. Employee was evaluated and diagnosed with a right upper arm contusion.

Supervisor was walking from the map case back to his truck and stepped in hole (7 inches) and fell to the ground. Supervisor was evaluated and was diagnosed with a right shoulder strain.

Miner was descending stairs from valve platform after loading water truck. He did not pay attention to foot placement and placed his right foot half on and half off landing platform at bottom of stairs. As he stepped forward, his right ankle rolled off of side of landing platform, approx. 2", and sprained right ankle.

Uneven ground/pot hole. Individual twisted his ankle and fell to the ground. The accident was not reported until the end of the shift. Initial doctor visit required no treatment or follow up. On 10/5/10, a return visit due to lack of ankle motion resulted in prescribed physical therapy.

Employee was working from the top step of a 3 foot ladder. When he attempted to descend, he stepped off back side of ladder instead of the front. This caused him to fall, striking his right elbow against a metal table, causing a one inch laceration as well as swelling and bruising.

EE was loading explosives into drill holes.Holes on the edge of the bench were on sloping ground so the ee was using a full body harness and lanyard for fall protection. EE lost his footing and slid app. 1' down the side of the bench, his slide was stopped by the fall arrest system. He slid, he reached up to grab the lanyard, which extended his left shoulder causing the injury.

Employee was descending a 6 foot step ladder when his left foot slipped off the bottom rung of the ladder and his right foot came in contact with the floor. EE alleged knee injury was related to this incident. Claim was contested and later accepted under workman's compensation. EE was severed during a plantwide workforce reduction on 5/19/08 and never returned to work.

Employee was removing chains from a compactor when he slipped in the mud. As he was falling, he reached out to steady himself and caught hold of the exhaust pipe of the compactor, sustaining 1st & 2nd degree burns to his right hand.

Attempted to enter the main quarry shop. The shop was locked. Attempted to unlock the door and was unsuccessful. Tried an alternate door. Entered and turned lights on. Before fully illuminated, he entered an unlit walkway. He tripped over a crate and struck a table or chair, causing upper lip lacerations. He was given 3 stitches and released to regular duty and no restrictions.

A mechanic was attempting to access the hinge area of a scaling unit to fix hydraulic oil leak. He exited the cab and chose a path that lead him over the exhaust system, fender and other components. Upon his final step to hinge area, which was soaked with oil, his right foot slipped from under him and he fell approx. 4' to the ground breaking the upper portion of his right arm.

In assessing a fall of ground, the employee instinctively jumped back when he heard more material fall (in this case it turned out to be ice). The employee fell backwards injuring his right knee - tore quadriceps tendon.

EE'S FOOT GOT TANGLED IN A WATER HOSE WHILE WASHING TRUCKS AND HE FELL. AT THAT TIME HE SAIDHE WAS NOT INJURED. ON 4-21-03, THE EE REPORTED BACK PAIN RELATED TO THE FALL THAT OCCURRED ON 3 -20-03. HE HAS BEEN DISABLED FROM WORK SINCE 4-21-03 UNTIL PRESENT.

THE EE WAS ROLLING UP AND PULLING 4" COLLAPSABLE WATER HOSE. WHILE DOING SO, HE WAS WALKING BACKWARDS AND TRIPPED OVER A LARGE ROCK FALLING TO THE GROUND. HE BROKE A BONE IN HIS RIGHT LEG AS A RESULT OF THE FALL.

AS HE STEPPED DOWN, THE WOOD BLOCK TURNED & EE FELL BACKWARDS & TURNED IN THE AIR LANDING ON HIS LEFT HAND & RIGHT WRIST.

USING A BOOM TRUCK, THE FINAL DR IVE WAS REMOVED, POSITIONED AT THE END OF THE TRAILER, AND BLOCKED TO PREVENT SIDE-TO-SIDE MOTION. AS THE CO-WORKER STEPPED OFF, HE FELL TO THE GROUND, LANDING IN UNEVEN AREA.
