Walking out of the training office employee tripped and fell down the stairs. Employees boot caught the second step and fell onto their knee 2 steps down pinning their left leg underneath.
The employee's left leg slid out and the right foot stayed planted. The employee caught self, but felt a pop in their right knee and maneuvered to the ground until emergency response was called.
Employee was demoing an out of service AC unit. Employee unbolted and removed the first section of the AC frame, set it on the floor, and reached for an impact. When they reached for the impact, they tripped and fell down to their left knee. Their left knee and shin landed on the piece of demo that was on the floor and cut their shin.
The employee was guiding the belt in along the west side walkway of the 27 conveyor when EE stepped onto some cross bracing where grating was removed and fell approximately 4' to the ground striking EE's face on the conveyor structure cutting EE's left cheek and damaging some teeth.
EE was replacing a valve. EE was using a torch to remove flange bolts, once EE was down to the last bolt, water that was trapped in the line started to spray out from around the flange. Not wanting to get wet, EE took a step backwards and tripped over a curb. EE put EE's hands out to catch self and bent EE's right wrist backwards, this resulted in a fracture of EE's wrist.
The employee was installing reflectors into berms and adding orange tape. While stepping back, the employee tripped over a rock and fell, landing on their left elbow, which resulted in a laceration.
Individual mis stepped while walking down a small flight of stairs and rolled an ankle resulting in a sprain/strain injury
Employee was traveling to sample pellets from line 4 and about the climb stairway from basement level, when they slipped on the pellets and mud at the base of stairway and twisted their left knee.
Employee was walking from Line 4 performing routine rounds when they slipped on pellets. The employee caught themselves & jaw came down and locked inflicting pain to lower jaw area. The employee notified the supervisor and EMS was called. The employee was then transported to telemedicine and later returned to work with a sore jaw and will be followed up on next shift per EMS
The employee tripped/slipped carrying a connection joint on a cable and landed on their left leg. The cable landed on the employee's leg as well.
The ee was working on the under grate of the line #3 pre-heat. There was a plywood work platform covering the 7-inch holes created by the flipper castings. The ee was pulling another sheet of plywood in to extend the platform, when ee's left foot stepped into the hole ee was planning to cover. This caused ee to drop to left knee, resulting in a laceration.
The incident occurred 30 minutes prior to the start of the employee's work shift. The EE was planning to unload tools from EE's car into the job trailer. When the employee was getting out of the car, EE slipped or tripped but never fell. The employee twisted to the right in an effort to regain balance, this caused a muscle strain.
While walking out to the service truck behind the weld shop by door 8, employee slipped on ice and fell catching self with right hand. Felt soreness in said hand immediately.
Conditions in the area were icy and snow covered. The slippery conditions caused the employee to slip and fall while walking to a work area. The incident resulted in a right hip and lower back strain.
Driver was stepping off the haul truck stairs onto the ground & twisted right knee & ankle. This became a recordable incident on 12/3/24 when the employee received restrictions.
EE was re-attaching the skirt board and liner bolts on the conveyor belt in the #1 Crusher, 6th level down. While pulling on a wrench tightening a bolt on the skirting board, EE lost EE's hold on the bolt and slipped forward, extending EE's left leg, and fell backwards. When EE slipped, EE attempted to catch self, but EE's left leg came out from under EE.
Employee descended stairs from flux tank catwalk to slurry tank catwalk, at the bottom of the stairs, while maneuvering around scaffolding, employee tripped and fell, striking the floor grating, causing lacerations to the chin
Employee was coming off the ladder at Step3 Coarse crusher. When EE got to the last rung to step down EE's left knee buckled under EE and EE fell from a standing position.
While walking down some grated steps in the fines crusher, employee walked through a door, and did not see the step. Employee lost balance, went down to the right knee, and felt a pop in upper left thigh area.
Employee working on filter head from step ladder. Vacuum pipe alongside ladder came loose from roustabout, rotated and slammed into ladder knocking employee off. Employee landed on EE's elbow. THIS INCIDENT BECAME A RECORDABLE ON SEPTEMBER 12th WHEN THE EMPLOYEE RECEIVED RESTRICTIONS.
EE was working to unclog Biomass Line 3 near the waste bin on east side of Agg. Step 2. EE stepped back on to debris that had fallen over the retaining wall and fell.
Injured right ankle stepping down from bucket after lighting welders welding torch. Did not treat at all in 2024. just recently went to the doctor. no restrictions or lost time
The EE was working from a 5' high scaffold deck, EE was tied off overhead with a self-retracting lifeline or SRL. While working, EE turned and stepped off of the deck, the SRL stopped the EE, and EE's right shoulder struck a steel column that was directly next to the scaffolding. Resulting in a dislocated shoulder. EE returned to work with temporary restrictions.
The employee had been working from a single high scaffolding platform. EE was coming down off of the scaffolding by way of an attached ladder. As EE stepped off the ladder onto the walkway below, EE rolled left ankle. This resulted in a closed fracture of the employee's left ankle.
While descending the production truck on a rest stop, driver felt knee twist on ice on the steps. The driver then ascended the stairs and felt the pain was such that it should be reported. It is undetermined the extent of ice on the stairs at the time of the incident.
Employee parked the 4252SS rock spreader truck. When the employee took the last step from the ladder to the ground the employee slipped and fell landing on left leg injuring ankle
MTE was going to walk downstairs from the 2nd floor to the first floor. There is a cement landing at the top of the stairway. As the employee stepped on the landing, EE's right knee gave out. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE STARTED PHYSICAL THERAPY ON 4/8/2024.
Employee was oiling a pneumatic impact and stepped over the bolt removal machine & caught their right boot on a hydraulic line. EE shook their boot to free the line, in the process reached out with their right arm against the mill housing to steady themself. At this point the employee felt/heard a crunching sound and immediately experienced discomfort in the right shoulder area.
Employee had just gotten down off the back of the cable reel truck. Employee turned to walk away from the truck. As the employee took a step with their left foot they stepped on a rock and fell to the ground. The employee stated they went to catch their fall with their left hand. The employee reported the incident to their supervisor.
Employee was coming down the stairs from the line 3 multi-clones. EE was finished checking the dust valves and heading to the main driveway in the basement. EE walked down the west side stairway towards the waste gas fan, then down onto the concrete landing where EE rolled EE's right ankle.
Three MTMs were reconnecting a crusher dust pipe (1/2" wall x 12" ID black rubber) to the crusher discharge chute. While pushing the pipe into place, it sprung to the north causing one of the employees to fall to the ground on left side and left wrist. The employee reported soreness in the left wrist.
Employee had stepped onto the hitch mounted platform to retrieve a shovel from the pickup box. While descending from the platform, the employee lost their balance and fell, landing on their right wrist.
Employee was descending a step ladder when EE's left foot slipped from the second rung from the bottom of the ladder. Injured employee slid down the ladder, catching self upright on left foot at ground level, with all the bodyweight on left leg and EE's knee locked. As a result the injured employee sustained an injury to the left knee.
THIS INCIDENT BECAME A RECORDABLE ON 06-02-2023 WHEN THE EMPLOYEE STARTED PHYSICAL THERAPY. Employee slipped on snow covered ice and fell while between 2 cars in the parking lot. The employee hit the ground on backside and experienced pain the right shoulder.
The employee stated that at approx. 9:45am EE pulled into a relief area. The employee got off the truck and took three steps and slipped on snow covered ice falling to the ground. The employee landed on their left front side and felt pain in the chest, ribs, shoulder, bicep, knee all on the left side. Emergency services took the employee to the dispensary for further evaluation.
Employee was working on haul truck that was parked. Employee was descending the stairs and their right boot caught the stair tread. The employee lost their balance causing to their left foot to twist in the process. The employee notified the operator on the truck to have them call Mine Control for Emergency Services to be dispatched to location.
Employee was exiting 4259 Haul truck at the end of the shift. Employee caught their heel on the bottom step of the ladder and fell to their knees on the ground. Employee stated that right knee landed on crushed rock which caused a laceration.
The EE was carrying a 4' x 4' piece of plywood to cover beams that had been painted when EE tripped and in EE's words "rolled ankle". EE fractured the 5th metatarsal bone of right foot. The injury did not involve mine equipment or system, job skills, proficiency, training, attitude or any protective items related to clothing or protective devices on equipment.
Employee was descending stairs and felt pain in the right knee. THIS INCIDENT BECAME A RECORDABLE ON 11/29/2022 WHEN THE EMPLOYEE RECEIVED A LIFTING RESTRICTION.
Employee was notified that water was backing up at a sump. The employee opened the dump valves to stop the water. When walking away, the employee's right leg stepped into an open launder that was covered with water. The employee attempted to grab a wall pipe with their left arm which caused a dislocation of the shoulder.
Employees finished clearing a rock jam in the #2B Feeder at the #2 primary mill conveyor tunnel. The rock jam had caused some spillage of rocks off the belt and into the walkway. Employee was walking up the tunnel when EE slipped on rocks in the walkway causing EE to fall to the ground and land on EE's right shoulder. Became a recordable on 9/8/22 when physical therapy started.
Employee was walking from the dry to the west stairwell on the second floor of the Learning Center when the employee tripped on the edge of a rug. It is believed that the employee attempted to catch themselves on the handrail of the stairwell, but was unsuccessful, and fell landing on their left shoulder where they felt pain.
Employee was adjusting cutter bar teeth on a balling drum. While working the the balling drum, the employee stepped into the opening of the scroll and fell, twisting their left knee.
The employee was walking into the Annex dry. While stepping with EE's left foot on the edge of the sidewalk, the employee rolled EE's ankle and fell to the ground.
MTM was working on the lime slurry mixing tank agitator. When descending the ladder, EE's foot slipped and EE put most of weight on EE's left arm and felt a pop in the bicep.
As operator was walking down the suitcase of a production truck, EE slipped on the bottom step. Employee caught self holding on to the hand railing and tweaked EE's right arm.
Employee was working in the steam room at MES cleaning a truck. Employee was standing on the 3rd step of a 4-step portable stairs. While descending the stairs, the employee missed the next step down and fell landing on right side.
Employee parked the 570 CAT 793F haul truck at the Mine Annex parking lot and was in the process of getting off of the truck. After employee deployed the Powerstep ladder they slipped with their right foot and fell down the Powersteps. The employee injured their lower back and both arms. The employee stated that the steps were slippery with mud at the time of the incident
Employee was operating the 374 CAT in the Pit. As employee was dismounting the dozer on the right side since the main boarding ladder was not functioning, employee fell approximately 3' to ground and landed on the right side of EE's body. The supervisor contacted emergency services to respond to incident. Restrictions were given on 2/24/2020 making this a recordable injury.
Employee was conducting a pre-shift inspection of the 545 production truck in the 65 relief area. As the employee was walking around the truck conducting the pre-inspection the employee slipped and fell on some ice hitting EE's head and back. The employee notified EE's supervisor and emergency services was notified.
