Walking out of the training office employee tripped and fell down the stairs. Employees boot caught the second step and fell onto their knee 2 steps down pinning their left leg underneath.

The employee's left leg slid out and the right foot stayed planted. The employee caught self, but felt a pop in their right knee and maneuvered to the ground until emergency response was called.

Employee was demoing an out of service AC unit. Employee unbolted and removed the first section of the AC frame, set it on the floor, and reached for an impact. When they reached for the impact, they tripped and fell down to their left knee. Their left knee and shin landed on the piece of demo that was on the floor and cut their shin.

The employee was guiding the belt in along the west side walkway of the 27 conveyor when EE stepped onto some cross bracing where grating was removed and fell approximately 4' to the ground striking EE's face on the conveyor structure cutting EE's left cheek and damaging some teeth.

EE was replacing a valve. EE was using a torch to remove flange bolts, once EE was down to the last bolt, water that was trapped in the line started to spray out from around the flange. Not wanting to get wet, EE took a step backwards and tripped over a curb. EE put EE's hands out to catch self and bent EE's right wrist backwards, this resulted in a fracture of EE's wrist.

The ee was working on the under grate of the line #3 pre-heat. There was a plywood work platform covering the 7-inch holes created by the flipper castings. The ee was pulling another sheet of plywood in to extend the platform, when ee's left foot stepped into the hole ee was planning to cover. This caused ee to drop to left knee, resulting in a laceration.

The incident occurred 30 minutes prior to the start of the employee's work shift. The EE was planning to unload tools from EE's car into the job trailer. When the employee was getting out of the car, EE slipped or tripped but never fell. The employee twisted to the right in an effort to regain balance, this caused a muscle strain.

Conditions in the area were icy and snow covered. The slippery conditions caused the employee to slip and fall while walking to a work area. The incident resulted in a right hip and lower back strain.

EE was re-attaching the skirt board and liner bolts on the conveyor belt in the #1 Crusher, 6th level down. While pulling on a wrench tightening a bolt on the skirting board, EE lost EE's hold on the bolt and slipped forward, extending EE's left leg, and fell backwards. When EE slipped, EE attempted to catch self, but EE's left leg came out from under EE.

The EE was working from a 5' high scaffold deck, EE was tied off overhead with a self-retracting lifeline or SRL. While working, EE turned and stepped off of the deck, the SRL stopped the EE, and EE's right shoulder struck a steel column that was directly next to the scaffolding. Resulting in a dislocated shoulder. EE returned to work with temporary restrictions.

The employee had been working from a single high scaffolding platform. EE was coming down off of the scaffolding by way of an attached ladder. As EE stepped off the ladder onto the walkway below, EE rolled left ankle. This resulted in a closed fracture of the employee's left ankle.

While descending the production truck on a rest stop, driver felt knee twist on ice on the steps. The driver then ascended the stairs and felt the pain was such that it should be reported. It is undetermined the extent of ice on the stairs at the time of the incident.

MTE was going to walk downstairs from the 2nd floor to the first floor. There is a cement landing at the top of the stairway. As the employee stepped on the landing, EE's right knee gave out. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE STARTED PHYSICAL THERAPY ON 4/8/2024.

Employee was oiling a pneumatic impact and stepped over the bolt removal machine & caught their right boot on a hydraulic line. EE shook their boot to free the line, in the process reached out with their right arm against the mill housing to steady themself. At this point the employee felt/heard a crunching sound and immediately experienced discomfort in the right shoulder area.

Three MTMs were reconnecting a crusher dust pipe (1/2" wall x 12" ID black rubber) to the crusher discharge chute. While pushing the pipe into place, it sprung to the north causing one of the employees to fall to the ground on left side and left wrist. The employee reported soreness in the left wrist.

Employee was descending a step ladder when EE's left foot slipped from the second rung from the bottom of the ladder. Injured employee slid down the ladder, catching self upright on left foot at ground level, with all the bodyweight on left leg and EE's knee locked. As a result the injured employee sustained an injury to the left knee.

The employee stated that at approx. 9:45am EE pulled into a relief area. The employee got off the truck and took three steps and slipped on snow covered ice falling to the ground. The employee landed on their left front side and felt pain in the chest, ribs, shoulder, bicep, knee all on the left side. Emergency services took the employee to the dispensary for further evaluation.

The EE was carrying a 4' x 4' piece of plywood to cover beams that had been painted when EE tripped and in EE's words "rolled ankle". EE fractured the 5th metatarsal bone of right foot. The injury did not involve mine equipment or system, job skills, proficiency, training, attitude or any protective items related to clothing or protective devices on equipment.

Employee was notified that water was backing up at a sump. The employee opened the dump valves to stop the water. When walking away, the employee's right leg stepped into an open launder that was covered with water. The employee attempted to grab a wall pipe with their left arm which caused a dislocation of the shoulder.

Employees finished clearing a rock jam in the #2B Feeder at the #2 primary mill conveyor tunnel. The rock jam had caused some spillage of rocks off the belt and into the walkway. Employee was walking up the tunnel when EE slipped on rocks in the walkway causing EE to fall to the ground and land on EE's right shoulder. Became a recordable on 9/8/22 when physical therapy started.

It is believed that the employee attempted to catch themselves on the handrail of the stairwell, but was unsuccessful, and fell landing on their left shoulder where they felt pain.

Employee parked the 570 CAT 793F haul truck at the Mine Annex parking lot and was in the process of getting off of the truck. After employee deployed the Powerstep ladder they slipped with their right foot and fell down the Powersteps. The employee injured their lower back and both arms. The employee stated that the steps were slippery with mud at the time of the incident

Employee was operating the 374 CAT in the Pit. As employee was dismounting the dozer on the right side since the main boarding ladder was not functioning, employee fell approximately 3' to ground and landed on the right side of EE's body. The supervisor contacted emergency services to respond to incident. Restrictions were given on 2/24/2020 making this a recordable injury.

As the employee was walking around the truck conducting the pre-inspection the employee slipped and fell on some ice hitting EE's head and back. The employee notified EE's supervisor and emergency services was notified.
