At approximately 11:55am, the Line 3 scrubber drop out box ruptured releasing large volumes of water onto the floor. The employee was likely north of the launder when the water swept them down the inclined walkway wedging them beneath the kiln pier stairs.

The injured was installing a swivel eye lifting device while EE's partner partially lifted the liner with a lining bar so EE could install the 3/4" nut underneath. The liner slid down the lining bar, pinching the injured's right middle and pointer fingers between the liner/nut and the pan feeder flights

Injured MTM was installing a drive sheave on a pump with a partner. The MTM's partner was installing the sheave while the injured MTM was pushing on the guard. The injured MTM's right pointer finger was in the pinch point of the guard hinge. When the injured MTM felt the pain from the guard pinching their finger, they pulled it out while it was still pinched.

EE was replacing a valve. EE was using a torch to remove flange bolts, once EE was down to the last bolt, water that was trapped in the line started to spray out from around the flange. Not wanting to get wet, EE took a step backwards and tripped over a curb. EE put EE's hands out to catch self and bent EE's right wrist backwards, this resulted in a fracture of EE's wrist.

MTMs were changing rollers on a belt. They were dragging a roller with a rope and decided to each carry on one end. The roller weighs about 150#. The injured carried the back and felt a pain go through their left elbow area. The employee reported the incident to their supervisor at the start of shift the next day (9-12-25).

2 MTMS were removing the outboard bearing on a dust collector. To free the bearing from the taper lock, the injured was using a 3lb. hammer while EE's partner held a chisel in a chisel holder against the bearing. During this process a piece of the chisel broke off and struck the injured resulting in a laceration to the left forearm.

61 was filling A fuel truck. They felt their back getting warm and saw that there was a fire. They exited the truck and ran a safe distance. Called the onsite fire department and also had the guard shack call the fire department. There were no injuries, and the truck was a total loss. Fire reported at 7:52 and MSHA was notified at 8:17 am.

The truck hit the strips and the employee was thrown forward and then pulled them back in the seat causing pain in the neck. EMS was called went to the ER for further eval. The truck was taken out of service so seatbelt could be checked and the road was graded. became recordable 6/10 with restrictions

A tree blew over into the power lines near the Basin Shops building at approx. 11:27 AM next to the road. Basin employees expelled 4 extinguishers and called Keetac Fire to respond. Keewatin fire department was called and the DNR and Nashwauk fire department. Basin cat was used to assist in cutting access. Fire was extinguished before 3 PM. MSHA was notified at approx. 11:56 am.

EE was using a cart to haul boiler tubes. The surface was smooth/flat concrete, slag/ash had fallen off the tubes while loading them on the cart. slag/ash prevented the wheels from freely moving in the intended direction while being pushed. This resulted in the cart tipping to one side. the EE stopped the cart from tipping, in doing so the EE strained the muscles of EE's left arm.

EE was removing steel grating from a floor launder for the purpose of cleaning the launder. As they were pulling the grating they experienced a pain in lower back on the right side. This incident resulted in the EE having a sprained lumbar.

EE was grinding/welding as part of bull gear repair that was taking place. They stopped grinding to visually check for high spots. When they flipped up their face shield, they felt something enter right eye. it was determined that the EE had a Corneal abrasion as a result of a small piece of grinding debris entering their eye.

The employee was originally trapped sometime after 11:30 PM on Saturday, April 19th. The employee was able to leave the elevator unharmed after the fire department responded and mechanically opened the doors
