Employee was performing maintenance on a screen deck. Upon completion of work the employee descended down the screen deck ladder. While descending the ladder the employee slipped on a rung and lost footing. To catch themselves from falling the employee caught themselves with left arm subsequently straining the arm and shoulder.

EE communicated with them ee was starting the plant. EE climbed of the belt and another ee walked 30 feet down the belt not expecting the belt to be turned on. The belt started and ee fell off about 4 feet
Employee slipped on rocks and fell while walking on the mine. No modified duty or loss time until 5/25/12 when employee was removed from work for surgery.
Our Customer (Black Diamond Trucking) had a passenger that exited the truck to make weight while on the scale. He jumped aprox 6 feet to an uneven surface. Upon impact, he broke his ankle and fell back hitting his head on the eye beam below, causing a laceration.

DESCENDING FROM A STEP STOOL. EE MISSED THE SECOND STEP, FELL BACKWARD, AND HIT HER HEAD ON THE EDGE OF THE DESK. EE'S IMPACT ON THE EDGE OF THE DESK CAUSED A 3/4" LACERATION TO THE BACK OF THE HEAD.
HE SLIPPED AND FELL WHILE WELDING ON TEXAS FEEDER AND DISLOCATED HIS RIGHT KNEE.
OPERATOR WAS MOVING A HYDRAULIC JACK AFTER ADJUSTING A CONE CRUSHER. HE SLIPPED ON AN OILY SURFACE ON THE CRUSHER PLATFORM, LANDING ON HIS BOTTOM LEFT SIDE.
