Contractor was changing crusher liners on an hp 400. While cleaning the seat of the liner the contractor's employee repositioned self with the grinder on. The grinder got pulled into employee's leg when the disc got caught up in employee's pants. The grinder was still running and cut employee's leg.

Driver hauling plaster sand got stuck in the soft sand, operator loader pushed the trailer out from being stuck.

Our Customer (Black Diamond Trucking) had a passenger that exited the truck to make weight while on the scale. He jumped aprox 6 feet to an uneven surface. Upon impact, he broke his ankle and fell back hitting his head on the eye beam below, causing a laceration.

THE OPERATOR TRAVELED DOWN A RAMP IN THE PIT. AT THE BOTTOM OF THE RAMP THE TRUCK HIT A POTHOLE IN STANDING WATER. THE DRIVER INJURED HIS LOWER BACK.
