EE was climbing a step ladder when EE's foot slipped and EE missed a step. To keep from falling, the EE grabbed on to a piece of machinery to steady themself. EE said that they felt like they may have pulled a muscle. The pain got worse over the next few days.

Showing all 18The EE was descending a ladder, lost grip and fell. The EE sustained injuries to the left arm (laceration and possible broken arm, laceration to the right thumb).
EE's were putting the hood on the crusher. Co-worker was operating the track hoe & lifted the hood up to crusher. When the hood got on top of crusher, it got stuck sideways & the chains came off. IE climbed up on the frame of the crusher & re-attached chains to the hood. IE tried to move hood from side to side. The hood popped up & hit IE's leg knocking IE 6' to the ground.
Dredge oper. was in the bottom of dredge checking the pump when he lost his footing on his right leg. When he came down on his right leg his knee was twisted or sprang.
Stepped off dredge to frozen ground. Rolled ankle over, bad sprain.
Employee stepped from scales to deck to clock in and in doing so turned his foot causing pain in his lower back.
EE WAS LEAVING BOILER THRU CHAIN CONVEYOR. HE STEPPED FROM TOP LEVEL ONTO BOTTOM SCAFFOLD BAORD. THE BOARD BROKE AND HE FELL 6 FEET INTO CHAIN CONVEYOR.
EE LOST HIS FOOTING WALKING ALONG RAILROAD TRESSELL GOING TO HIS CAR.
EMPLOYEE WAS GETTING INTO LOADER TO FILL ROCK BIN. HE LIFTED RIGHT LEG TOO HIGH TO REACH FIRST STEP, BUMPED RIGHT KNEE ON SECOND STEP OF LOADER.PAIN WAS SO BAD HE PASSED OUT FALLING INTO FLOOR OF LOADER. HE BUMPED HIS HEAD AND RIGHT SIDE AS HE FELL.
EE WAS STEPPING OVER PIPE & CAUGHT HIS FOOT & FELL LANDING ON HIS RIGHT HAND. EE WAS DISTRACTED & WAS NOT PAYING ATTENTION TO HIS FOOTING.
EE STEPPED ON A PIECE OF AROMA FLEX (BLOCK FROM INSULATION), THAT WAS PARTLY UNDER WATER AND HADMUD ON IT. EE SLIPPED AND FELL ON HIS RT. HIP, WHEN HE FELL HE HIT THE LOCKER ALONG THE POWER HOUSE AND WALL.
THE AREA WHERE EE FELL WAS SLIPPERY WITH ALGAE GROWING, BECAUSE OF A STOPPED UP DRAIN.
DO NOT KNOW EXACTLY EE FELT PAIN WHEN HE WAS WALKING DOWN FRON E-40 BAGHOUSE.
EMPLOYEE WAS STANDING ON THE FENDER OF THE LOADER WASHING THE WINDOW. HE WAS HOLDING ON TO SIDE MIRROR WITH ONE HAND. HE LOST HIS BALANCE AND FELL TO THE GROUND. HE WAS COMPLAINING OF A HURT B ACK AND SORE LEG.
CLIMBING DOWN BETWEEN BOILER AND CATWALK. EE PUT HIS KNEE ON A ROUND PIECE OF SKIM CAUSING TO GET HIS FOOTING BELOW WHEN HE FELT HIS KNEE POP.
EE SLIPPED WHEN HE PLACED HIS FOT ON THE EDGE OF A HOPPER & BUMPED HIS SHIN. EE DID NOT REPORT INJURY UNTIL 4/29. DR.'S NOTE DID NOT INDICATE DIAGNOSIS. WE WERE UNABLE TO DETERMINE (THRU DR.) UNTIL 5/9/00.
EE STEPPED IN A POT HOLE.
EE WAS WALKING OUT OF FIRST AID CLASS WHEN SOMEONE STUPPED IN FRONT OF HIM AND HE STEPPED WRONG, TWISING HIS KNEE.
