Employee was checking screens when EE fell and scraped arm on grading.
On 2/2/2020 the miner reported to the safety supervisor that the miner had fallen while dismounting dump truck on 1/31/2020. Miner stated miner mis-stepped one of the steps causing miner to fall to the ground at a height of approximately 3ft.
Sprained knee
It was lightly raining and an employee slipped on the top of the steps and fell down 4 steps. While trying to catch self, employee twisted back.
Employee climbed up short vertical ladder to look at belts on elevator. When descending ee stepped off ladder to platform wrong and twisted ankle.
Miner was injured while moving a conveyor. The tail pulley end raised off of the ground slightly picking the miner up and allowing ee to fall about 2 feet back to the ground spraining rt ankle and left knee. Restricted duty case, follow up in a week. Miner may return to restricted duty on 05/25/2017.
A customer truck driver was rolling up the tarp when the customer fell through a wooden board on the trailer that was rotted.
There was an electrical fire at the variable frequency drive in the MCC room at #2 Wet Plant. EE began to extinguish the fire using a fire extinguisher. EE then ran to the main disconnect located at the transformer station to turn off the power. While running, ee stepped on uneven ground and injured knee.
Employee was moving a water hose and stepped on uneven packed sand. Pulled muscle in right calf.
Employee stepped off of the concrete slab with his left leg. He knee buckled, and he fell down.

Employee stepped off the stacking platform onto the floor and lost his footing. In an effort to keep from falling, he grabbed the forklift which was parked nearby. He did keep from falling but he hurt his shoulder in the process.
Employee was stepping onto the top of a hopper car to get clay sample and he hit his left shoulder on the handrail. He then lost his balance and grabbed the handrail with his left hand and felt a severe pain in his left shoulder.
EE was moving pneumatic tanker after loading and grabbed handle on side of truck to enter drivers cab, handle was wet with dew and hand slipped. EE fell hitting right inner thigh on truck step and left knee on ground.
EE WAS ASSISTING IN CHANGING A CABLE ON THE DREDGE FROM THE WORKBOAT. EE STEPPED OVER THE SEAT, SLIPPED AND SCRAPED RT. LEG - SHIN.
EMPLOYEE WAS DISMOUNTING THE FRONT END LOADER IN11-99 AND INJURED HIS LEFT SHOULDER BUT NO MEDICAL ATTENTION WAS SAUGHT AT THE TIME. RECENTLY HIS SHOULDER BEGAN TO BOTHER HIM AND MEDICAL ATTEN TION WAS SAUGHT AND SURGERY WAS DONE ON LEFT ROTATOR CUFF ON 5-17-00.
