Had some water from melting snow that had leaked on this inside and employee had slipped.

Based on employee's account of this, employee was stepping off the step ladder when EE's right foot slipped off the bottom step. This caused EE's right toes to bend and when the right foot / toes hit the ground, all EE's weight was on EE's right foot / toes. Employee did not mention it to EE's supervisor until the afternoon of 1/26/2026.

Employee sprained right ankle when getting off of forklift at propane tank.

A pump had stopped working so water had built up on the floor causing a slippery surface on a step of the drill.

Employee was attempting to check coolant on loader which resulted in coolant being sprayed on face and hands. EE then jumped of work platform and injured ankle/knee.

Employee was cleaning out a chute with a shovel when EE slipped hitting EE's right side of EE's abdomen on a handrail. EE was seen 1 week later at doctor.

EE was walking to the work area, when EE twisted and sprained their right ankle. EE was walking on dry concrete coming down an incline.

Employee was walking from the dryer to the mill. They slipped on the floor and hit their right knee on a metal post, causing it to swell. The employee was shutting everything down because of lightening in the close proximity of the plant.

Employee was walking out of the plant to put jacket and lunchbox in vehicle before clocking out. The grass and crush & run were wet between the plant and the employee parking lot. The employee slipped in that area and was falling backwards. Employee extended right arm to catch fall. As a result, the employee's right elbow was dislocated.

The employee has no clear description of how the incident happened he did several jobs that day and at no time can he recall bumping, twisting, or hyper extending the knee. When Workers' Compensation carrier had a conversation with him he said he might have done it by slipping on an incline and then jumping to keep balance but never fell.

Employee slipped on steps, resulting in a bruised elbow and back pain.

Employee loss balance when climbing down out of a front end loader. The employee landed on left shoulder and scraped left knee. The employee thought his shoulder was o.k, he went to his doctor on 11/15/12 and the doctor gave restrictions of *avoid lifting overhead and *avoid lifting over 25lbs until 12/5/12.

Employee was coming down a second flight of stairs in crusher area near #3 belt and missed the last step. The employee strained lower right hand side of back. Employee thought his back was o.k. until he scheduled a doctors appointment on July 17, 2012, and the doctor has scheduled physical therapy to start July 20, 2012.

Employee tripped and fell on a welding lead, bruising his right shoulder

Employee was walking outside when he slipped on a snow covered patch of ice. Employee broke lower right leg, he was taken to the emergency room. Employee was scheduled to have surgery 2/1/2010.

Employee said that on 1-26-09 he was walking down icy steps when his left foot slipped outward. Employee stated he was holding the railing when his foot slipped and he felt pain in his lower back. He told his foreman about the incident on 1-27-09. He has been seen by a Chiropractor 3 times, 2-4,2-5 & 2-6-09. No Lost Time.

Employee said he was walking down the ramp toward #2 packer area, when he slipped and fell on the damp floor. He landed on the metal ramp bracing, bruising his left thigh. He was seen by DR Moon on 12-3-08, sent for x rays and given prescriptions. No Lost Time.

Employee stated that he fell on an icy patch, twisting his right leg underneath him. He complained of back pain and was taken to the Emergency Room for evaluation. Employee lost 1 work day and returned to work 11-3-08. He began losing time again on 11-5-08. (Dr slip off work until further notice)

Employee was on top of a bulk truck, properly harnessed with a belt-type harness, but slipped off truck and was suspended by his harness. The employee overfilled truck and kaolin clay was on top of truck on and around walkway. Conditions were dry.

Employee was standing on the front of a CAT 950 loader working on a clamshell feeder and slipped, falling approximately 6 feet. He fractured his knee and had superficial cuts on his forehead

Employee stated that he was walking across a roof walkway when he slipped on wet talc twisting his left knee and bumping his left elbow. He saw a Doctor on 2-15-08 and was prescribed therapy for a possible cartilage problem with his left knee. No lost time.

Employee was proceeding to help another worker bar down hung up material when slipped on uneven ground. He twisted his back but thought it was okay. On 4-13-07 the employee went to a chiropracter for an adjustment.

Employee was climbing up the steps of D-30-D ore truck when he slipped on the ice. He fell bumping his left elbow and right knee. As he was falling he grabbed the hand rail and pulled his rig ht shoulder. On 6/19/2007 the employee first saw a Dr about shoulder pain and on 7/3/2007 was put on lifting restrictions. No lost time yet.

Employee was coming down a ladder, slipped and twisted his left ankle.

Foreman stopped to pick up EE from location where he was working as a front-end loader operator for a ride to lunchroom for dinner. EE climbed down out of Cat 966 loader to get into pickup and slippped on icy surface. Bruise back.

Icy conditions on ramp leading up fom bottom of pit

SNOWY/ICY CONDITIONS, AS EMPLOYEE CLIMBED DOWN FROM FRONT-END LOADER.

EE WAS GETTING OFF A FORKLIFT WHEN HE STEPPED ONUNEVEN PAVEMENT AND SPRAINED HIS RIGHT ANKLE.

EMPLOYEE WAS WALKING ON THE SIDEWALK NEAR THE PACKING OFFICE. HE DIDN'T REALIZE IT, BUT HE WAS TOO CLOSE TO THE EDGE OF THE SIDEWALK. HIS LEFT FOOT SLID OFF THE SIDEWALK AND HIS ANKLE TWISTED.

THE EMPLOYEE WAS WALKING DOWN THE RAMP IN THE PACKING AREA WHEN HIS FOOT/ANKLE TWISTED CAUSING HIS RIGHT ANKLE TO SWELL.

EE WAS DESCENDING STAIRS BY BIG SWECO SCREEN. HESLIPPED ON STAIR TREAD AND SLID DOWN APPROXIMATELY 2-3 STAIRS ON HIS HEEL, THEN CAUGHT HIMSELF ON RAIL. HE WENT TO PHYSICIAN ON 11/19/01.

IT WAS RAINING THAT NIGHT. THE EE WAS WALKING BETW3EEN THE MILL AND THE DRYER. CONDITIONS WERE WET AND THE EE SLIPPED AND FELL. HE INJURED HIS BACK. HE BRUISED MUSCLES SURROUNDING THE SCAR TIS SUE FROM A PREVIOUS SURGERY.

EE CAME DOWN FROM ROOF, GOT BOOTS WET FROM WATER ON ROOF. CARRYING STEEL PLATE DOWN STAIRS EE SLIPPED AND FELL HITTING LEFT SIDE ON HANDRAIL.
