Employee was in a man basket preparing to load a round when a fall of roof occurred. The rock hit the man basket causing the employee to be jostled inside. Employee sustained scrapes on left shoulder, laceration to right bicep/Left leg, and contusion on employee's head.

Employee was stripping power cable to install a plug, the knife slipped and cut into the left-hand thumb causing it to cut to tendon

Employee was pulling on a wrench while working on the jumbo drill and felt something pull in left shoulder. The employee did not get medical treatment when it first happened. EE decided to make an appointment with a doctor for February 23, 2016 because the shoulder was not getting better. Employee is scheduled for 4-6 weeks of therapy.

The employee has no clear description of how the incident happened he did several jobs that day and at no time can he recall bumping, twisting, or hyper extending the knee. When Workers' Compensation carrier had a conversation with him he said he might have done it by slipping on an incline and then jumping to keep balance but never fell.
