Showing all 6Employee was pulling into a shot at 5:30. He stepped down both of the steps outside the truck, went left foot first towards the ground and rolled on his ankle. It was dark outside so he could not tell if a rock caused the ankle to roll. He went to the doctors for x-rays. The ankle was sprained.
EE was standing on the 48" discharge conveyor belt of a McCloskey 154 crusher. He removed the cover from the radiator hydraulic cooler. EE placed the cover on top of the crusher. He stepped to the left with left leg causing left knee to give way.

EE WAS WORKING ON IMPACT CRUSHER CHANGING OUT BLOW BARS, HE CLIMBED ON SIDE OF CRUSHER WITH ONE FOOT ON CRUSHR AND THE OTHER ON A HANDRAIL. HE TURNED HIS BODY CAUSING THE FOOT ON THE HANDRAIL TO SLIP. EE FELL HITTING HIS LEFT RIB CAGE AGAINST HANDRAIL.
