Injured worker was cutting a cleated conveyor belt using a utility knife when the blade reached the thicker part of the belt, the blade skipped causing the blade to cut worker's right leg above knee

Injured worker was using a wrench and hammer trying to remove a nut. When the nut gave way, the worker's hand was smashed between the wrench and the bearing housing EE was working on. Injured worker received three stitches between thumb and index finger of left hand.

While helping to clean up build up around return belt pulley, as EE pulled up on the pry bar injured worker felt a shooting pain in groin area. Injured worker has an internal hernia as diagnosed by physician.

Injured worker was using a sledge hammer to break a lodged rock very close to a handrail, as EE swung the hammer the hammer hit the metal handrail bouncing the sledge back hitting worker in the head. Worker received stitches in head, no loss of consciousness was reported.

Injured worker was attempting to take out a larger rock from inside the crusher when his hand slipped and smashed is right index finger against the metal of the crusher. Worker was wearing leather gloves when accident occurred.

EE bent over to flip the floor panel (4x8 plywood per phone call) over when he dropped his end and could not stand up due to pain in his lower back. Back strain. (Per phone call, EE will see a specialist soon for further diagnosis).

Employee was walking under a conveyor belt, stepped in mud and twisted ankle. The employee worked remainder of the shift, then went to doctor the next day. Employee was diagnosed with a broken bone in right ankle.

Employee was pulling into a shot at 5:30. He stepped down both of the steps outside the truck, went left foot first towards the ground and rolled on his ankle. It was dark outside so he could not tell if a rock caused the ankle to roll. He went to the doctors for x-rays. The ankle was sprained.

EE was standing on the 48" discharge conveyor belt of a McCloskey 154 crusher. He removed the cover from the radiator hydraulic cooler. EE placed the cover on top of the crusher. He stepped to the left with left leg causing left knee to give way.

Employee was seen in the parking lot bent over and sick at his stomach. Employee was pale, weak and complaining of chest pain. Decision made to transport to doctor. An ambulance was called and transported employee to hospital. Initial diagnosis determined employee had suffered a mild heart attack. Employee is recovering at this time.

EE WAS WORKING ON IMPACT CRUSHER CHANGING OUT BLOW BARS, HE CLIMBED ON SIDE OF CRUSHER WITH ONE FOOT ON CRUSHR AND THE OTHER ON A HANDRAIL. HE TURNED HIS BODY CAUSING THE FOOT ON THE HANDRAIL TO SLIP. EE FELL HITTING HIS LEFT RIB CAGE AGAINST HANDRAIL.

STOCKPILING BASE MATERIAL WITH A CAT 631B SCRAPER; OPERATOR DROVE OFF SIDE OF STOCKPILE. SCRAPER ROLLED OVER ON ITS RT. SIDE. STOCKPILE HEIGHT APPROX. 6'; OPERATOR'S LEG HIT STEERING WHEEL, CAUSING A BRUISE TO HIS L LEG ABOVE KNEE.
