IE was unloading boxes from the tailgate of a pickup truck and carrying them into a Connex storage container. As IE stepped up into the container, EE's foot slipped and EE's right leg came down hard onto the ground causing a sprain to EE's right knee.

During the fall, the employee attempted to regain balance by grabbing the door, which caused EE's body to shift to the right. Upon impact, EE landed on EE's right elbow and reported pain.

While walking down the haul truck stairs, the operator tripped and fell, resulting in a laceration to the knee.
Mechanic was climbing down ladder of a under ground haul truck when EE stepped off the ladder on to the ground EE's knee twisted. EE had a small tear in EE's meniscus. EE worked full duty until the date of EE's surgery in August. it took a a while to schedule because it was out of state and EE had to lose weight before the surgery.
As Contractor security officer was walking down handicapped ramp, they stepped off the bottom of the metal ramp onto the gravel and rolled their ankle and fell back onto their tailbone.

EE was working near a conveyor belt when they tripped over their own feet which resulted in a fall towards the conveyor belt. During the fall, the EE's right arm made contact with the belt and went between the belt and the roller. This incident resulted in a fracture to the EE's right arm. The E-stop pull cord was activated stopping the conveyor.
Miner was descending out of the cab of a jumbo and when EE stepped onto the ground EE hyperextended EE's knee resulting in a torn ACL and meniscus. Miner was able to work until EE's pre-op appointment on 10/10/2024
A piece of falling equipment caused an employee to swiftly move out of the way. As the employee started to run, EE stepped on an uneven ground surface and fell. The initial x-rays reveal a fracture to the left heel.
While employee was walking from work area, they observed that the area needed to access had icy ground conditions. In an attempt to navigate through the ice, employee proceeded to shuffle slowly. Employee lost footing and fell backwards resulting in a fractured patella.
Injured EE without Fall Protection and the knowledge of the crew members, moved to the second deck and fell through an open hole used to lift items to the second deck. Fell to the ground with damage to EE's pelvis, Left Shoulder and ribs.
When an underground mechanic's foot slipped off the step as they were getting off a haul truck, they caught themselves using both hands and their other foot. They felt their right bicep pop as a result of doing this. They might have a torn ligament. Until they see an orthopedic physician the next week, they were put on light duty.

After leg came free, employee fell to the ground. X-ray revealed a fracture at the end of the femur bone where the ligament attaches above the right knee.

The employee just wanted to get it documented at this time and went back to work Knee was sore. 4-25-23 same injury 1 year later no mechanism of injury this time, went to the doctor and employee will need Surgery to replace old ACL hardware and install new hardware.
Employee was moving a pallet jack on the portal pad when EE slipped and fell. EE had some light pain in lower back at this time. This was not a reportable at the time of injury. EE still felt pain on 4-18 and decided to get a cortisone shot and was feeling sick so just wanted to go home to recover. EE got a note from doctor.
A miner operating a 472 haul truck (CAT 789 model) was exiting the cab, the operator's heel hooked on the door seal causing EE to fall backwards, hitting left elbow and forearm on radiator. EE was taken to the local hospital for further evaluation; where it was found that EE sustained a fracture to the left Ulna proximal to the elbow.
Loose gravel on wash out platform contributed to poor footing.
Employee was changing hose in the back of a fuel pick up truck, when the fitting broke lose EE lost EE's balance and stumbled back falling out of the back of the truck. EE's pelvis has a light fracture.
I.P. was installing Insulated Metal Panels when I.P. lost their footing. As I.P. fell they grabbed on to the edge of the Panel that was going to be installed. The grabbing motion at the edge / corner of the panel tore I.P.'s glove and therefore cutting the right thumb inside the glove.
Employee was walking down the road by the pound at Comet water treatment plant carrying a spool of rope. As EE was walking EE slipped on some ice falling and twisting ankle. There was gravel on the road and EE was wearing ice cleats.
Mechanic was doing a PM on a 1500 haul truck and was on the top deck. The heel of the mechanic's boot caught the expanded metal when the mechanic was moving backwards. The mechanic fell back hitting head on the corner of the radiator shroud. The employee had a small gash on the back of head.

A mobile maintenance PM mechanic was walking on a concrete wash pad with a hose in hand when they slipped, landing on their right elbow and straining the right shoulder.

Miner slipped on Kabota tractor platform while mounting the vehicle. When the miners foot slipped it got caught between the platform and engine.
An employee slipped and fell on icy ground. Employee worked employee's scheduled shifts and full duty until March 5th when employee did not report to employee's scheduled shift.
EE was ascending the ladder to the grizzly feeder at the crusher. EE boots were muddy and ee lost footing on the rungs of the ladder and fell backwards about 5 feet to the floor below, landing on tail bone.
On 5 January at ~10:00 AM an underground haul truck operator twisted EE's ankle when stepping off an underground tractor. When the incident occurred the operator was moving the tractor that was located in a bay where EE wanted to stage EE's haul truck. When EE was climbing off the tractor EE stepped on uneven ground twisting EE's left ankle.
A driller walking on the pattern at night stepped into a drill hole and twisted EE's knee. After medical treatment and observation, it was determined EE needed surgery. EE missed EE's next scheduled shift on March 8th.
